粉尘螨滴剂脱敏治疗鼻炎并腺样体肥大患儿的临床分析

摘　　要：目的将粉尘螨滴剂应用于过敏性鼻炎并腺样体肥大患儿脱敏治疗,分析其床疗效及免疫调节功能。方法选取在医院接受治疗的过敏性鼻炎并腺样体肥大患儿98例,随机分为试验组与对照组,每组各49例。对照组予以常规方案;试验组予以对照组相同方案的同时,加用粉尘螨滴剂治疗。对比两组治疗后总症状评分(鼻塞、流涕、喷嚏等)、腺样体大小、血清IL-2及IL-6变化、尘螨特异性IgE(sIgE)、尘螨总IgE(T-IgE)、尘螨特异性IgG4(sIgG4)变化。结果治疗8 w后,两组总症状评分、腺样体A/N比值、sIgE、T-IgE、sIgG4均优于治疗前(P<0.05),且试验组总症状评分、腺样体A/N比值、sIgE、T-IgE均明显低于对照组(P<0.05),而s IgG4明显高于对照组(P<0.05)。结论采用粉尘螨滴剂脱敏治疗过敏性鼻炎并腺样体肥大患儿,可更有效缓解鼻塞、喷嚏等症状,缩小腺样体,有一定免疫调节疗效,有助于患儿恢复。Objective To apply Dermatophagoides Farinae Drops to allergic rhinitis and adenoid hypertrophy in children in order to analyze the clinical efficacy and immune and regulatory function. Methods A total of 98 children with allergic rhinitis and adenoid hypertrophy to receive treatment in our hospital were selected and divided randomly into an experiment group and a control group （n=49, respectively）. The control group received conventional treatment while the experiment group was administered with Dermatophagoides Farinae Drops on the basis of the treatment for the control group. The total symptom scores（nasal congestion, runny nose, and sneezing, etc.）, the size of adenoids, and the changes in serum IL-2 and IL-6, dermatophagoides specificity IgE （sIgE）, total IgE （T-IgE）, and dermatophagoides specificity IgG4 （sIgG4） were compared between the two groups after the treatment.Results Aftereight week＇s treatment, the total symptom scores, adenoid ratio A/N, sIgE, T-IgE, and sIgG4 in both groups were better than those before the treatment （P 〈 0.05）, and the total symptom scores, adenoid ratio A/N, sIgE, and T-IgE in the experiment group were significantly lower than those in the control group （P 〈 0.05）, while sIgG4 was significantly higher than that in the control group （P 〈0.05）.Conclusion In the treatment of allergic rhinitis and adenoid hypertrophy, Dermatophagoides Farinae Drops can effectively relieve nasal congestion, sneezing and other symptoms, reduce the adenoids, have certain immune regulation effect, and are helpful for children recovery.
