摘  要:目的:在静息状态下,观察不同性别海洛因依赖者默认模式网络(DMN)低频振幅的变化。方法:被试者包含26名海洛因依赖者(HD组)和20名健康对照者(HC组)。其中,HD组来自复旦大学附属华山医院心理咨询门诊、上海交通大学医学院附属瑞金医院临床心理科门诊和上海交通大学医学院附属上海市精神卫生中心,男性14名,女性12名。HC组来自复旦大学附属华山医院体检中心。所有被试者完成一般人口学和成瘾相关资料后,进行功能磁共振扫描获取静息态磁共振数据。一般人口学及成瘾相关资料采用SPSS 16.0软件进行统计学分析,fMRI数据使用SPM8软件分析。结果:HD组和HC组被试者均完成一般人口学和成瘾相关资料的评估。因为明显头动,HD组2例和HC组1例磁共振数据被剔除。两组被试者的年龄、受教育年限及性别构成比差异均未见统计学意义(P>0.05)。静息态低频振幅的结果显示,与HC组相比,HD组患者的左侧扣带回后部、右侧中央旁小叶、双侧额上回内侧、双侧颞中回及右侧楔前叶的低频振幅活动明显减弱。与女性相比,男性HD患者的缘上回、角回及额上回内侧脑区的低频振幅活动增强;但右侧楔前叶、左侧颞上回、左侧颞中回和左侧中央旁小叶的ALFF活动则减弱。结论:海洛因依赖改变了大脑DMN的功能,这种变化存在着性别差异。Purpose: To explorer the gender difference of amplitude of low frequency fluctuations (ALFF) in patients with heroin dependence. Methods: Twenty-six patients with heroin dependence (HD group) and 20 age and gender matched health controls (HC group) were recruited. All patients came from the out-patient departments of psychiatry department in Huashan hospital of Fudan university and of clinical psychiatry department in Ruijin Hospital Affiliated to Shanghai Ji the physical examiaotong University and Shanghai Mental Health Center Affiliated to Shanghai Jiaotong University. HC group participants came fromnation department of Huashan hospital. All participants were screened by mini international neuropsychiatric disease interview (MINI) . Patients who met DSM-IV diagnostic criteria for heroin dependence without comorbidity (except nicotine addiction), aged 18-45 years old, right handed were enrolled. All participants were assessed by Hamilton anxiety scale (HA/vIA) and Hamilton depression scale (HAMD). The above assessment and evaluation were implemented by two professional psychiatrists. After completion of MRI scans for all subjects, the unqualified data due to head movement and other factors were excluded. SPSS16.0 was used to analyze the demographic data and related information about addiction. Functional imaging data were processed by SPM8 and REST toolbox. Results: The all participants finished data collection. Totally, 3 functional data (2 in HD group and 1 in HC group) were moved out of the analysis due to head movement. The result demonstrated that the total scores of HAMA and HAMD, and numbers of cigarette smoking were much higher in HD group than that in HC group (t=-12.434, P= 0.000; t=-9.390,P= 0.000, respectively).
