过敏性紫癜患者治疗前后其外周血TGF-β和IL-6在纯真CD4^+T细胞分化为Th17细胞和调节性T细胞过程中变化

摘　　要：目的:观察以疏风清热、活血化瘀立法治疗过敏性紫癜(HSP)前后,外周血转化生长因子β(TGF-β)和白细胞介素6(IL-6)在纯真CD4+T细胞(Naive T)分化为辅助性T细胞17(Th17)和调节性T细胞(Treg)的过程中的变化。方法:使用流式细胞仪检测36例住院患者治疗前后和健康对照组外周血Th17、Treg的水平,使用实时荧光定量PCR仪检测TGF-β、IL-6和转录因子ROR-γt的水平,比较其差异。结果:(1)HSP患者治疗后与治疗前(急性期)相比较,IL-6和Treg的水平升高(P<0.05),Th17和ROR-γt的水平降低(P<0.05),TGF-β的水平无明显差异(P>0.05);(2)HSP患者治疗前与健康对照组相比较,Th17和ROR-γt的水平明显升高(P<0.05),IL-6和Treg的水平降低(P<0.05),TGF-β的水平无明显差异(P>0.05);(3)HSP患者治疗后与健康对照组相比较,IL-6和Treg的水平升高(P<0.05),Th17和ROR-γt的水平降低(P<0.05),TGF-β的水平无明显差异(P>0.05)。结论:以疏风清热、活血化瘀立法治疗过敏性紫癜,能明显干预患者的外周血TGF-β、IL-6、ROR-γt、Th17及Treg的水平,对患者免疫功能的调节作用明确,临床效果显著,极大的提高了临床诊疗能力。Objective：To observe the therapeutic effect of dispelling wind and clearing heat,promoting blood circulation to remove blood stasis legislation in treatment of allergic purpura（HSP）,the peripheral blood transforming growth factor β（TGF-β）and interleukin-6（IL-6）in pure CD4^＋ T cells（Naive T）differentiation of T helper 17（Th17）and regulatory T cells（Treg）in the process of change. Methods：Using flow cytometry to detect 36 cases of hospitalized patients before and after treatment and health control group,peripheral blood Th17 and Treg level,using the real time fluorescence quantitative PCR detection of TGF-β and IL-6 and transcription factor ROR-γt level,compare the differences. Results：（1）HSP therapy in the treatment of anterior（acute period）,IL-6 and Treg levels were elevated（P〈0.05）and Th17 and ROR-γ T levels decreased（P〈0.05）. TGF-β levels had no significant difference（P〉0.05）.（2）HSP treatment of healthy group,Th17 and ROR-γt levels were significantly increased（P〈0.05）,IL-6 and Treg levels decreased（P〈0.05）. TGF-β levels had no significant difference（P〉0.05）.（3）HSP treatment of healthy group,IL-6 and Treg level increased（P〈0.05）,Th17 and ROR-γt levels decreased（P〈0.05）. TGF-β levels had no significant difference（P〉0.05）. Conclusion：Dispelling wind and clearing heat,promoting blood circulation to remove blood stasis legislation in treating allergic purpura,can significantly interfere with the outer peripheral blood of TGF-β and IL-6,the ROR-γt,Th17 and Treg levels,the patients＇ immune function regulation clear,clinical effect significantly,greatly improves the ability of clinical diagnosis and treatment.
