温肾活血化湿方联合激素治疗原发性肾病综合征临床研究

摘　　要：目的:观察温肾活血化湿方联合激素治疗原发性肾病综合征的临床疗效。方法:纳入76例脾肾阳虚型原发性肾病综合征患者,随机分为2组各38例,对照组给予激素治疗,观察组在激素治疗基础上给予温肾活血化湿方治疗。比较2组的临床疗效,观察治疗前后中医证候评分、肾功能指标[血尿素氮(BUN)、肌酐(SCr)]、血清白蛋白(Alb)、24 h尿蛋白定量、血脂指标[甘油三酯(TG)、总胆固醇(TC)]的变化。结果:治疗后,观察组完全缓解率、总有效率均高于对照组(P<0.01)。2组中医证候积分均低于治疗前(P<0.05),观察组中医证候积分低于对照组(P<0.05)。2组BUN、SCr均低于治疗前(P<0.05),但2组2项指标比较,差异均无统计学意义(P>0.05)。2组Alb、24 h尿蛋白定量均较治疗前改善(P<0.01),观察组2项指标的改善均优于对照组(P<0.05,P<0.01)。2组TC、TG值均低于治疗前(P<0.01),观察组2项指标值均低于对照组(P<0.05,P<0.01)。结论:在激素治疗基础上采用温肾活血化湿方治疗脾肾阳虚型原发性肾病综合征患者,可有效改善患者的肾功能,减少尿蛋白,调节血脂,缓解症状明显,疗效确切。Objective：To observe the clinical effect of Wenshen Huoxue Huashi prescription combined with hormone for primary nephrotic syndrome（PNS）.Methods：Selected 76 PNS patients of Yang deficiency of spleen and kidney type,and divided them into two groups randomly.The control group was treated with hormone,and the observation group was given Wenshen Huoxue Huashi prescription based on the treatment of the control group.Compared clinical effect of the two groups,observed change of Chinese medicine syndrome score,renal function[blood urea nitrogen（BUN）,serum creatinine（SCr）],serum albumin（Alb）,24 hours urinary protein],Blood lipid indexes[triglyceride（TG）,total cholesterol（TC）].Results：After treatment,the complete response and the total effective rate of the observation group were higher than those of the control group（P〈0.01）.Chinese medicine syndrome scores of the two groups were both lower than those before treatment（P〈0.05）,and the Chinese medicine syndrome scores of the observation group were lowerthan those of the control group（P〈0.05）.BUN,SCr of the two groups were lower than those before treatment（P〈0.05）,but no significant difference was found when the two indexes of the two groups were compared（P〈0.05）.Compared with before treatment,Alb and 24 hours urinary protein of the two groups were improved（P〈0.01）,and improvement ofthe two indexes in the observation group were betterthan that in the control group（P〈0.05,P〈0.01）.Values of TC and TG of the two groups were lowerthan those before treatment（P〈0.01）,and values of the two indexes in the observation group were betterthan those in the control group（P〈0.05,P〈0.01）.Conclusion：Wenshen Huoxue Huashi prescription can improve renal function,increase plasma AIb and reduce urinary protein and blood lipid in treating PNS patients of Yang deficiency of spleen and kidney type.
