玉屏风散加味治疗慢性荨麻疹表虚不固证的抗变态反应作用研究

摘　　要：目的:探讨玉屏风散加味治疗慢性荨麻疹表虚不固证的抗变态反应作用。方法:将慢性荨麻疹患者130例随机分为2组各65例。对照组给予西替利嗪口服液,试验组给予玉屏风散加味方治疗。疗程为4周。记录治疗前后风团数目、风团大小、瘙痒程度、持续时间、皮肤划痕征、水肿程度等症状、体征评分;检测治疗前后Th1细胞因子[干扰素-γ(IFN-γ)、肿瘤坏死因子-α(TNF-α)]和Th2细胞因子[白细胞介素-2(IL-2)、IL-4、IL-10]水平。记录复发情况。结果:试验组临床疗效总有效率为93.85%,高于对照组的81.54%,2组比较,差异有统计学意义(χ~2=4.561,P<0.05)。与治疗前相比较,治疗后2组患者血清IFN-γ、IL-2和IL-10均升高,TNF-α和IL-4水平均下降(P<0.01),治疗后试验组IFN-γ、IL-2和IL-10水平高于对照组,TNF-α和IL-4水平低于对照组(P<0.01)。随访12周,试验组复发6例,复发率9.23%;对照组复发18例,复发率27.69%,试验组复发率低于对照组(χ~2=7.358,P<0.01)。结论:玉屏风散加味治疗慢性荨麻疹表虚不固证,临床疗效显著,且复发率低,并能调节Th1/Th2细胞因子。Objective：To observe the effect of modified Yupingfeng san on anti- allergy for chronic urticaria with weakness exterior-qi syndrome.Methods：Divided 130 cases of patients with chronic urticaria into two groups randomly,65 cases in each group.The control group was treated with levo- cetirizine hydrochloride oral solution,while the treatment group was treated with the prescription of modified Yupingfeng san.The two groups both received treatment for a course of four weeks.Recorded the symptoms,such as the wheal number,size,pruritus,persisting time,erythema and edema degree as well as scores of physical signs before and after treatment.Detected levels of Th1 cytokines[interferon-γ（IFN-γ） and tumor necrosis factor（TNF-α）]and Th2 cytokines[interleukin- 2（IL- 2）,IL- 4 and IL-10].Recurrence was also recorded.Results：The clinical total effective rate of the test group was 93.85%,being higher than 81.54%of the control group（χ^2= 4.561,P〈0.05）.Comparing with those before treatment,levels of IFN-γ,IL- 2 and IL-10 in two groups were all increased,and levels of TNF-α and IL- 4 in two groups were both decreased after treatment（P〈0.01）.After treatment,in the test group,levels of IFN-γ,IL- 2 and IL- 10 were higher than those in the control group,and levels of TNF-α and IL-4 were lower than those in the control group（P〈0.01）.In the follow-up of 12 weeks,6 cases of recurrence were observed in the test group with a recurrence rate of 9.23%;18 cases of recurrence were observed in the control group with a recurrence rate of27.69%.The recurrence rate of the test group was lower than that of the control group（χ^2=7.358,P〈0.01）.Conclusion：The therapy of modified Yupingfeng san on anti- allergy for chronic urticaria with weakness exterior- Qi syndrome has curative effect and low recurrence rate,and it can also regulate Thl/Th2 cytokines.
