摘 要:目的探讨血清胱抑素C(Cys C)对于冠心病患者心血管事件的预测价值是否独立于肾功能。方法共有305例患者入组,主要统计Cys C、超敏C反应蛋白((hs-CRP)、肾小球滤过率(e GFR)和冠脉积分,以血清Cys C水平为标准,按四分位数分四组,然后做相关性分析。结果 1冠心病组Cys C水平(0.83±0.35)显著高于对照组(0.72±0.24),两组比较,差异具有统计学意义(P<0.01)。2冠状动脉病变严重程度评分随血清Cys C水平的升高而逐渐加重。3 Cys C与炎症指标hs-CRP、纤维蛋白原相关。4 Cys C是独立于e GFR的冠心病的预测指标。结论本研究认为Cys C作为一种理想的反映GFR变化内源性标志物,与冠心病有明显相关性,是独立于e GFR,并且提示炎症是联系Cys C与CAD的重要因素。Objective The aim of this study was to investigate the role ofcystatin C level as a predictor for cardio- vascular events in patients with coronary artery disease(CAD) regardless of kidney function. Methods Three hundred and five coronary artery patients were included according to the CAD severity score into this study. Serum eystatin C levels, high sensitive C-reactive protein (hs-CRP)level, estimated glomerular filtration rate (eGFR) and the CAD severity score were calculated. Serum Cys C level was divided into four groups by quartiles Results (1)Cys C level (0.83 ±0.35) of the CHD group was significantly higher than that of the control group (0.72 ±0.24), (P 〈 0.01)..(2) The severity score of coro- nary artery lesion gradually increased with the level of serum Cys C.(3There was a positive correlation between cystatin C and CRP or fibrinogen levels.(4)Cys C independent of eGFR, was a predictor for coronary heart disease.Conclusion Cys- tatin C could be a useful laboratory biochemical marker in predicting the severity of CAD independent of eGFR. Cystatin C level is associated with biochemical atherosclerosis markers such as serum CRP and fibrinogen levels.
