176例突发性耳聋患者的临床特征分析

摘　　要：目的探讨突发性聋(突聋)发病的相关危险因素,为临床预防和治疗提供参考依据。方法回顾性分析176例突聋患者与60例健听对照者的临床资料。结果本研究突聋患者男女性别比为1.02:1;以冬季(31.82%)和春季(27.84%)为高发季节;发病多见于19~59岁的中青年(85.23%),尤其是青年患者(48.30%);95.45%的患者伴有耳鸣、眩晕等耳部症状;突聋类型多见于平坦下降型(38.50%,72/187)和全聋型(27.27%,51/187);提示OSAHS、糖尿病、电磁辐射是突聋的危险因素,与对照组比较差异具有统计学意义(P<0.05);睡眠时间、高血压病、高血脂、上感病史、精神心理因素及高盐饮食是突聋的高危因素,与对照组比较差异具有显著统计学意义(P<0.01)。结论突发性聋以中青年、冬季发病多见,其发病与多种因素相关,临床诊疗应注重全身各系统综合评估,多学科系统治疗。Objiective To investigate risk factors of idiopathic sudden sensorineural hearing loss, and provide references for prevention and treatment accordingly. Methods Various clinical parameters were analyzed in 176 patients with idiopathic sudden sensorineural hearing loss and 60 healthy people as the control. Results The gender ratio of male and female was 1.02.31.82% of the patients fell ill in the winte time, and 27.84% in the spring.The incidence of idiopathic sudden sensorineural hearing loss was common in the patients aged 19 to 59（85.23%）,especially in the youger（48.30%）.95.45% of the patients bothered with ear symptoms, such as tinnitus, dizziness, etc. Hearing loss was more common in the gradually sloping（38.50%,72/187） and total deafness（27.27%,51/187）.OSAHS, diabetes, electromagnetic radiation are the risk factors of sudden sensorineural hearing loss,and there are statistical differences between the contral group and the hearing loss group（P〈0.05）.Sleeping hours, hypertension, hyperlipidemia, upper respiratory tract infection, psychological factors and high salt diet are the high risk factors of sudden sensorineural hearing loss,showing the statistical differences between the contral group and the hearing loss group（P〈0.01）. Conclusions The incidence of idiopathic sudden sensorineural hearing loss is mainly in the younger and middle aged in winter. It is related to multiple factors. The thorough and comprehensive assessment of all body systems and multidisciplinary systematic care is helpful for clinical diagnosis and treatment.
