中医药联合早期肠内营养用于胃癌根治术后30例疗效观察

摘　　要：目的 :探讨中医药联合早期肠内营养对胃癌根治术患者术后康复、炎性反应和免疫功能的影响。方法 :60例患者随机均分为观察组和对照组。对照组术后采取早期肠内营养,观察组术后给予新斯的明足三里注射+肠内四物汤+早期肠内营养,比较两组术后康复(肠鸣音恢复时间、首次排气时间、首次排便时间和住院时间)、炎性反应(IL-6、TNF-α)和免疫功能(Ig A、Ig M、Ig G和CD4+/CD8+)指标。结果 :观察组肠鸣音恢复时间、首次排气时间、首次排便时间和住院时间明显少于对照组(P<0.05)。观察组术后7 d的IL-6、TNF-α浓度显著低于对照组(P<0.05)。观察组术后7 d的Ig A、Ig M、Ig G和CD4+/CD8+明显高于对照组(P<0.05)。结论:中医药联合早期肠内营养可有效促进胃癌根治术患者术后康复,抑制炎性反应,提高免疫功能。Objective：To explore the effect of traditional Chinese medicine combined with early enteral nutrition on postoperative rehabilitation,inflammatory reaction and immune function in patients treated with radical surgery for gastric cancer. Methods：Sixty patients with gastric cancer were randomly divided into the obser- vation group and the control group. After the operation,patients in the control group were treated with early enteral nutrition,while in the observation group patients were given neostigmine injection into zu san //（ST 36） plus enteral Siwu decoction plus early enteral nutrition. Postoperative rehabilitation（restoration time of bowel sounds,anus exhaust and defecation and hospitalization duration）,inflammatory reaction index（lL-6 and TNF-α） and immune function index（IGA,IgM,IgG and CD4＋/CD8＋） were compared between the two groups. Results：Restoration time of bowel sound ,anus exhaust and defecation,hospitalization duration in the observation group were significantly less than those in the control group（P〈0.05）. The contents of 1L-6 and TNF-α 7 days after operation in the observation group were significandy lower than those in the con- trol group（P〈0.05）. The levels of IgA,IgM,IgG and CD4＋/CD8＋ 7 days after operation in the study group were significantly higher than those in the control group（P〈0.05）. Conclusion ：Traditional Chinese medicine combined with early enteral nutrition could effectively promote postoperative recovery,inhibit inflammatory re- action and improve immune function in patients with radical operation for gastric cancer.
