硒酵母对Graves病^(131)I治疗后TPO-Ab和TRAb的观察

摘　　要：目的:观察Graves病患者在^(131)I治疗后补充硒酵母后血液中TPO-Ab和TRAb的变化。方法:将241例Graves病患者随机分为两组。对照组(120例)如发现甲减或亚临床甲减,予雷替斯(左甲状腺素片)处理;如果甲亢仍严重,予甲巯咪唑口服治疗;两种情况下,均结合血液甲状腺功能检查,使血液中TSH水平降至正常值下限附近。观察组(121例)在对照组的基础上^(131)I治疗3个月后加硒酵母片口服。记录^(131)I治疗Graves病满3、9个月时血液中FT3、FT4、TSH、TPO-Ab、TRAb水平。结果:3个月时对照组与观察组血液中TPO-Ab和TRAb滴度无明显差异(P>0.05)。两组TPO-Ab和TRAb滴度9个月较3个月时明显降低,差异有统计学意义(P<0.05);9个月时观察组较对照组中TPO-Ab和TRAb滴度明显降低(P<0.05)。结论:^(131)I治疗Graves病后使用硒酵母有助于降低TPOAb和TRAb滴度,降低甲减的发生率。Objective：To observe the changes of serum thyroid peroxidase antibody（TPO-Ab）and thyroid stimulating hormone receptor antibody（TRAb）in the blood of patients with Graves disease after iodine 131 therapy and enriched Selenium yeast.Methods：241patients with Graves disease were randomly divided into two groups.Control group（120people）if we found hypothyroidism or subclinical hypothyroidism,we should treat the patients with thunder.If the hyperthyroidism is still severe,we should treat the patients with methimazole oral;in the two cases,it is combined with the blood thyroid function test,so that the concentration of thyroid stimulating hormone in the blood is reduced to the lower limit of the normal value.Observation group（121people）on the basis of the control group,3months after the treatment of iodine,give each patient to oral Selenious Yeast Tablets.We recorded FT3,FT4,TSH,TPO-Ab,TRAb concentrations at 3 months,9 months after Graves disease treatment.Results：There was no significant difference in TPO-Ab,TRAb between the control group and the observation group at 3months（P〉0.05）.In the two groups,the levels of TPO-Ab,TRAb in 9months were lower than the 3months,there were significantly differents（P〈0.05）.9months after the observation group than in the control group,TPO-Ab,TRAb concentrations were significantly reduced（P〈0.05）.Conclusion：The use of selenium yeast after 131 I in the treatment of Graves disease is helpful to reduce the concentration of TPO-Ab and TRAb,and to reduce the incidence of hypothyroidism.
