血清IL-6和IL-8与TNF-α联合检测在胎膜早破早期宫内感染预测中的价值研究

摘　　要：目的研究血清IL-6、IL-8与TNF-α联合检测在胎膜早破早期宫内感染预测中的价值,为胎膜早破早期宫内感染的诊断及防治提供参考。方法选取2014年6月-2015年12月医院收治40例胎膜早破早期宫内感染者为A组,40例胎膜早破未发生早期宫内感染者为B组,40例健康孕妇为C组,然后比较三组血清IL-6、IL-8与TNF-α水平,并比较A组与B组足月与未足月者的检测结果、A组不同严重程度绒毛膜羊膜炎者的检测结果。结果 A组血清IL-6、IL-8与TNF-α水平分别为(13.56±1.42)pg/ml、(15.78±1.69)pg/ml与(2.29±0.24)ng/ml,均高于B组(10.04±1.20)pg/ml、(12.53±1.42)pg/ml、(1.66±0.18)ng/ml及C组的(6.42±0.67)pg/ml、(7.85±0.78)pg/ml、(0.91±0.13)ng/ml,B组的检测水平则高于C组,同时A组足月与未足月者的检测结果均高于B组,A组不同严重程度绒毛膜羊膜炎者的检测结果比较,差异有统计学意义(P<0.05)。结论血清IL-6、IL-8与TNF-α联合检测在胎膜早破早期宫内感染及其严重程度的预测价值较高。OBJECTIVE To study the value of combined detection of serum IL-6, IL-8, and TNF-α in prediction of early intrauterine infection due to premature rupture of membranes so as to provide guidance for diagnosis, prevention and treatment of the early intrauterine infection. METHODS Totally 40 patients with early intrauterine infection clue to premature rupture of membranes who were treated in hospitals from Jun 2014 to Dec 2015 were chosen as the group A, 40 premature rupture of membranes patients without early intrauterine infection were assigned as the group B, and 40 healthy pregnant women were set as the group C. The levels of serum IL-6, IL-8, and TNF-α were compared among the three groups, the test results were compared between the patients with full-term gestation and the patients without full-term gestation in the group A and the group B, and the test results were com- pared among the patients with different severities of chorioamnionitis in the group A. RESULTS The levels of serum IL-6, IL-8, and TNF-α of the group A were respectively （13. 56±1.42）pg/ml, （15.7±1. 69）pg/ml, and （2.29±0.24）ng/ml,higher than （10. 04±1.20）pg/ml, （12. 53±1.42）pg/ml, and（1. 66± 0. 18）ng/ml of the group B as well as （6.42±0.67）pg/ml, （7.85±0.78）pg/ml, and （0.91±0.13）ng/ml of the group C; the levels of the above indexes of the group B were higher than those of the group C, meanwhile, the levels of the indexes of the pregnant women with or without full-term gestation were higher in the group A than in the group BI there was significant difference in the test result among the patients with different severities of chorioamnionitis in the group A （P〈0.05）. CONCLUSION The value of combined detection of serum IL-6, IL-8, and TNF-α is high in the prediction of the early intrauterine infection due to premature rupture of membranes and its severity.
