水面舰艇官兵超长期护航的异常心理状况研究

摘　　要：目的分析水面舰艇官兵赴亚丁湾执行超长期护航任务期间的异常心理状况,为开展针对性的干预提供依据。方法采用症状自评量表(Symptom Checklist-90,SCL-90),分别于出航前7天和远航第77天、第142天、第204天,对执行远航任务的某水面舰艇官兵进行"重复性横断面"团体问卷调查和异常心理状态的筛查分析。结果 SCL-90总分的阳性筛查显示:出航前舰员的亚健康状态(总分值>160分)、中度心理问题(总分值>200分)以及高度心理问题(总分值>250分)的筛查阳性率分别为2.35%、1.18%和0%。远航第77天和第204天的亚健康状态筛查阳性率(分别为15.05%和20.00%)均明显高于出航前水平,前后比较差异具有统计学意义(P<0.01)。远航第204天的中度和高度心理问题筛查阳性率(分别为8.00%和4.80%),与出航前比较差异具有统计学意义(P<0.05)。SCL-90因子分的阳性筛查显示:与出航前比较,远航第77天突出表现在"强迫症状、忧郁、敌对性、其他因子",远航第142天的异常心理主要表现在"偏执",而远航第204天则突出表现在"躯体化、敌对性、偏执、其他因子"。结论超长期护航期间舰员呈现的异常心理状况,早期有明显的波动和调整适应过程,6个月后呈现症状加重,需实施心理干预或休整。Objective To investigate the mental status of navy servicemen on surface ship during super long-time voyage in Gulf of Aden,and provide the basis for targeted prevention and support.Methods Reproducibility and cross sectional survey was conducted to investigate the mental status of navy servicemen on a surface ship with Symptom Checklist-90（SCL-90）tests,separately at 7 th day before deployment,and the 77 th,142 th and 204 th day after deployment during super long-time voyage.Results The screening positive rates of sub-health state（SCL-90 total score〉160）,moderate psychological problems（total score〉200）and severe psychological problems（total score〉250）were 2.35%、1.18% and 0%,respectively.Both of the screening positive rates of sub-health state at 77 th day（15.05%）and 204 th day（20.00%）were significantly higher than those before deployment（P〈0.01）.Statistical differences of the screening positive rates of moderate psychological problems（8.00%）and severe psychological problems（4.80%）were observed as compared with those of before deployment（P〈0.05）.Psychological problems were obsessive-compulsive symptoms,depression,hostility,and other factors at 77 th day after deployment.Paranoid was the prominent factor of psychological abnormalities at 142 th day after deployment,the outstanding performance of somatization,hostility,paranoia and other factors were found at 204 th day after deployment.Conclusion During super long-time voyage,navy servicemen on surface ship exhibit complex abnormal mental status,with fluctuations and adaptation process in the prophase.Six months later,psychological problems deteriorates sharply,and navy servicemen need for rest or psychological intervention.
