子宫肌瘤检出率与高血压、高血糖、血脂异常的关联性研究

摘　　要：目的基于北京市2014年医院体检数据,以医院体检人群为单位分析子宫肌瘤检出率与高血压、高血糖、血脂异常(以下简称"三高")之间的关联性。方法选择对体检人群均进行子宫肌瘤检查、"三高"测量的北京27所医院,共242 331人,计算各医院子宫肌瘤、"三高"的检出率,分析其相关性,应用主成份分析法提取"三高"检出率的主成份,将子宫肌瘤检出率经logit变换后作为因变量,以年龄、提取的主成份为自变量,拟合多水平模型。结果子宫肌瘤检出率为11.45%,高血压、高血糖、血脂异常的人群检出率分别为9.55%、6.23%、28.97%。50岁以下,子宫肌瘤检出率与"三高"检出率的双变量Spearman秩相关系数均在0.6~0.7之间(均为P<0.05);主成份和多水平线性模型分析结果显示子宫肌瘤检出率存在医院差异,并随"三高"整体水平的上升而升高。结论子宫肌瘤检出率与高血压、高血糖、血脂异常检出率正相关,并随着"三高"检出率整体水平上升而升高。Objective To explore the correlation between detection rate of uterine fibroids and hypertension, hyperglycemia and dyslipidemia based on the data of hospital＇s checkup in Beijing in 2014. Methods A total of 242331 females from 27 hospitals in Beijing were chosen for a medical checkup, and the data of their uterine fibroids, hypertension, hyperglycemia and dyslipidemia were examined. Principal component analysis and multilevel linear model were used in the multiple-factor analysis. Results The detection rate of uterine fibroids, hypertension, hyperglycemia and dyslipidemia were respectively 11.45%, 9.55%, 6.23% and 28.97%. The spearman correlation coefficients of uterine fibroids and hypertension, hyperglycemia and dyslipidemia were 0.6 to 0.7 （P〈0.05） for those aged under 50, and 0.214 （PM）.05）, 0.256 （P〈0.05）, 0.508 （P〈0.05） for those aged over 50, respectively. The detection rate of uterine fibroids varied with specific hospitals, and positively correlated with overall of detection rates of hypertension, hyperglycemia and dyslipidemia （Principal componentl）. Conclusion Our analysis showed that the detection rate of uterine fibroids is positively correlated with the overall detection rates of hypertension, hyperglycemia and dyslipidemia.
