摘　　要：目的探讨T2DM患者血清25-羟维生素D[25(OH)D]水平与颈动脉内-中膜厚度(CIMT)的相关性。方法选取896例T2DM患者,测定血清25(OH)D水平、CIMT以及HbA_1c、血脂、肝肾功能、血钙、高敏C反应蛋白(hsC-RP)等指标,根据血清25(OH)D水平的四分位数将患者分为4组,比较4组间CIMT差异,分析影响CIMT的相关因素。结果25(OH)D水平较低组的CIMT高于25(OH)D水平较高组[(0.98±0.13)vs(0.96±0.13)vs(0.93±0.13)vs(0.91±0.12)mm,P<0.05]。多元逐步回归分析显示,CIMT影响因素为年龄、25(OH)D、糖尿病病程、hsC-RP、吸烟、BMI、LDL-C(β分别为0.691、-0.627、0.356、0.217、0.190、0.162、0.132,P<0.05或P<0.01)。结论在T2DM患者中,CIMT的增加与血清25(OH)D的缺乏相关。Objective To explore the association between serum 25-hydroxyvitamin D [25（OH）D ]and carotid intima-media thickness（IMT）in patients with type 2diabetes mellitus（T2DM）. MethodsA total of 896 patients with T2 DM were recruited.Serum 25（OH）D,Carotid IMT,HbA1 c,blood lipid,hepatic and renal function,calcium,and high sensitive-C reactive protein（hsC-RP）were measured.All the patients were divided into four quartile subgroups according to serum 25（OH）D level.Carotid IMT were compared among four subgroups and the correlation factors were analyzed. Results Carotid IMT was significantly higherin subjects with low 25（OH）D level than in subjects with high 25（OH）D level[（0.98±0.13）vs（0.96±0.13）vs（0.93±0.13）vs（0.91±0.12）mm,P〈0.05].Multiple stepwise regression analysis indicated that age,25（OH）D,the duration of diabetes,hsC-RP,smoking,BMI,and LDL-C were influencing factors for carotid IMT（standardized regression coefficients was 0.691,-0.627,0.356,0.217,0.190,0.162 and 0.132,P〈0.05 or P〈0.01）. Conclusion In patients with T2 DM,serum 25（OH）D deficiency is associated with an increase in carotid IMT.
