摘  要:目的分析非外伤性结肠破裂多层螺旋CT(MSCT)表现,以提高对该病的认识和诊断水平。方法回顾性分析经手术病理证实的13例非外伤性结肠破裂患者的临床及MSCT资料,均行腹盆部MSCT平扫,其中7例行MSCT增强扫描,分析非外伤性结肠破裂的MSCT表现特征。结果结肠癌6例,憩室炎4例,自发性穿孔3例;CT表现为腹腔游离气体6例,肠腔外局限性小气泡征7例,肠壁连续性中断4例,肠外粪块5例,肠系膜、网膜脂肪密度增高4例,腹膜腔或腹膜后积液6例。结论 MSCT能较好的诊断非外伤性结肠破裂及显示破裂部位,对非外伤性结肠破裂有较高的诊断价值。Objective To analyze the multi-slice spiral CT( MSCT) manifestations of non-traumatic rupture of colon,so as to improve the understanding and diagnostic level for this disease. Methods The clinical and MSCT data of 13 cases confirmed as non-traumatic rupture of colon by surgical pathology were retrospectively analyzed. All patients underwent unenhanced abdominopelvic MSCT,amongst these 7 cases underwent enhanced abdominopelvic MSCT. The MSCT manifestation of non-traumatic rupture of colon was analyzed. Results Among the 13 cases of non-traumatic rupture of colon,there were 6 cases of colon cancer,4 cases of diverticulitis and 3 cases of spontaneous perforation. As for the CT manifestation,free intraperitoneal air was shown in 6 cases,extra-luminal air bubble sign was displayed in 7 cases,interruption of intestinal wall continuity in 4 cases,extra-colonic feces with peritoneal contamination in 5 cases,increased density of mesentery and omental fat in 4 cases,and peritoneal or retroperitoneal effusion was demonstrated in 6 cases. Conclusion MSCT can adequately diagnose non-traumatic rupture of the colon and reveal the location of rupture. MSCT has a high diagnostic value in non-traumatic rupture of colon.
