超声和X线检查诊断先天性肥厚性幽门狭窄的价值

摘  要:目的探讨超声和X线检查诊断先天性肥厚性幽门狭窄的价值。方法回顾分析32例经手术证实为CHPS患儿的超声和X检查结果。结果28例行超声检查中25例(89.3%)幽门环肌厚度≥4.0mm,幽门管长度范围12.0-25.0mm,幽门管横断面外径范围12.5—21.0mm;32例X线造影检查均可见胃排空延迟,胃扩张30例,幽门鸟嘴征、线样征29例,胃窦肩征23例,蕈伞征16例。结论CHPS具有特征性的影像学表现,超声和X线检查对CHPS的早期诊断都有很高的参考价值,超声可作为有明显临床表现患儿筛查的首选检查方法,X线上消化道造影典型的影像学征象对CS的进一步检查非常重要。Objective To investigate the value of ultrasound and X-ray examination in diagnosis of congenital hypertrophic pyloric stenosis(CHPS). Methods A retrospective analysis of the ultrasound and X-ray examination results of 32 CHPS children cases, confirmed by operation. Results 28 cases examined by ultrasound, the pyloric ring muscle thickness≥4.0mm in 25 cases(89.3%), the length of pyloric canal range of 12.0-25.0mm, the peripheral diameter of pyloric canal range of 12.5-21.0mm; the gastric emptying time was delayed in all 32 cases, gastic dilatation sign was seen in 30 cases, pyloric string sign and pyloric angle sign were captured in 29 cases, sinus ventriculi shoulder sign was showed in 23 cases, mushroom sign was seen in 16 cases. Conclusion Expression of CHPS has characteristic imaging, ultrasonography has a high sensitivity and specificity for the diagnosis of CHPS, X-ray examination has very high clinical value in the diagnosis of CHPS.
