摘 要:目的探讨维生素E治疗早期糖尿病足的临床效果及对氧化应激反应和炎症指标的影响。方法选择2015年2月~2016年2月于延安大学附属医院内分泌科接受治疗并符合纳入标准的糖尿病足患者110例,依据随机数字表法分为对照组和实验组,每组55例。对照组接受常规糖尿病足治疗,实验组在常规治疗基础上予以早餐后维生素E 100 mg/d口服。观察两组的临床疗效,比较两组治疗前后的空腹血糖、餐后2 h血糖、糖化血红蛋白、晚期蛋白氧化产物(AOPPs)、总抗氧化能力(TAOC)、超氧化物歧化酶(SOD)、丙二醛(MDA)、血管细胞黏附分子-1(VCAM-1)、成纤维细胞生长因子2(FGF2)、肿瘤坏死因子-α(TNF-α)及白细胞介素-6(IL-6)水平。结果实验组有效率显著高于对照组(P<0.05)。两组治疗后空腹血糖、餐后2 h血糖水平均显著低于治疗前(P<0.05)。两组治疗后AOPPs、MDA均低于治疗前,TAOC、SOD均高于治疗前,差异有统计学意义(P<0.05);实验组治疗后AOPPs、MDA均低于对照组,TAOC、SOD均高于对照组,差异有统计学意义(P<0.05)。两组治疗后TNF-α、FGF2、IL-6及VCAM-1均低于治疗前,实验组较对照组降低更加显著,差异有统计学意义(P<0.05)。结论维生素E治疗早期糖尿病足的效果较好,可显著改善氧化应激反应及炎症情况。Objective To study the clinical effect of Vitamin E in the treatment of early diabetic foot and the influence on oxidative stress and inflammation index. Methods 110 cases of treated and met inclusion criteria patients with early diabetic foot from February 2015 to February 2016 in Department of Endocrinology, Affiliated Hospital of YaM'an University were selected. The patients were divided into control group and experimental group according to random number table method, 55 cases of each group. Conventional diabetic foot treatment was used in control group, Vitamin E (100 mg/d, oral) was applied in experimental group on the basis of routine treatment. Clinical effect between two groups was observed. Fasting blood glucose, postprandial 2 h blood glucose, glycosylated hemoglobin, advanced oxidation protein products (AOPPs), total antioxidant capacity (TAOC), superoxide dismutase (SOD), malondialdehyde (MDA), vascular cell adhesion molecule-l.(VCAM-1), fibroblast growth factor 2 (FGF2), tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6) levels before and after treatment between two groups were compared. Results Effective rate of experimental group was significantly higher than that of control group (P 〈 0.05). After treatment, fasting blood glucose, postprandial 2 h blood glucose levels in two groups were lower than before treatment (P 〈 0.05). After treatment, AOPPs, MDA levels in two groups were lower than before treatment, TAOC, SOD levels in two groups were higher than before treatment, with statistical differences (P 〈 0.05). After treatment, AOPPs, MDA levels in experimental group were lower than those in control group, TAOC, SOD in experimental group were higher than those in control group, with statistical differences (P 〈 0.05). After treatment, TNF-α, FGF2, IL-6, VCAM-1 levels in two groups were lower than before treatment, experimental group was significantly lower than control group, with statistical differences (P 〈 0.05).
