摘  要:目的探讨体位及物理振动在肾下盏结石行体外冲击波碎石(ESWL)术后的应用价值。方法选取2014年10月~2016年11月经嘉兴市第一医院ESWL碎石治疗后的肾下盏结石患者99例,将其随机分为对照组(50例)和观察组(49例)。对照组在ESWL术后采用多饮水、增加运动量等自然排石方法,结石直径10~15 mm;观察组在ESWL术后采用倒立体位、敲击肾区物理振动排石治疗及自然排石方法 ,结石直径10~15 mm。观察两组排石疗效、不良反应和并发症。结果观察组当天排石率为59.2%,2周后结石排净率为63.3%,对照组当天排石率为32.0%,2周后结石排净率为36.0%,两组比较差异有统计学意义(P<0.05)。两组患者均无严重并发症发生。观察组血尿、肾绞痛、输尿管石街并发症发生率分别为79.6%、28.6%、18.4%,对照组血尿、肾绞痛、输尿管石街并发症发生率分别为76.0%、24.0%、16.0%,两组比较差异无统计学意义(P>0.05)。结论倒立体位及肾区物理振动可明显促进肾下盏结石ESWL治疗后的结石排出,且安全无创,值得临床推广应用。Objective To investigate the application value of body position and physical vibration after extracorporeal shock wave lithotripsy (ESWL) for subrenal calyx calculus. Methods Ninety-nine patients with subrenal calyx calculus treated by ESWL in the First Hospital of Jiaxing City from October 2014 to November 2016 were selected, and they were randomly divided into control group (50 cases) and observation group (49 cases). The control group was taken the natural expelling calculi method by drinking water, adding exercise and so on after ESWL, the calculi diameter was 10-15 ram. The observation group was taken the expelling calculi method of physical vibration by inverted position and artificial knock on renal region, and the natural expelling calculi method, the calculi diameter was 10-15 ram. The ex- pelling calculi effects, adverse reactions and complications of the two groups were observed. Results The lithagogue rate of the first day in the observation group was 59.2%, the stone-free rate after 2 weeks in the observation group was 63.3%; the lithagogue rate of the first day in the control group was 32.0%, the stone-free rate after 2 weeks in the con- trol group was 36.0%, the differences between the two groups were statistically significant (P 〈 0.05). There were no serious complications in both groups. The incidence of hematuria, renal colic and ureter stone-street in the observation group was 79.6%; 28.6%, 18.4% respectively; the incidence of hematuria, renal colic and ureter stone-street in the control group was 76.0%, 24.0%, 16.0% respectively, there were no significant differences between the two groups (P 〉 0.05). Conclusion The method of inverted position and artificial physical vibration in renal region after ESWL on sub- renal calyx calculus can significantly promote the lithecbole, and it is safe, noninvasive and worthy of clinical promo- tion and application.
