早期系统康复治疗在胫骨平台Schatzker Ⅳ～Ⅵ型骨折环形外固定术后对膝关节功能的影响

摘　　要：目的探讨早期系统康复治疗在胫骨平台Schatzker Ⅳ~Ⅵ型骨折环形外固定术后对膝关节功能的影响。方法选择哈励逊国际和平医院2013年7月~2015年7月80例胫骨平台Schatzker Ⅳ~Ⅵ型骨折并进行骨折环型外固定术的患者,按照随机数字表法分为观察组和对照组,每组各40例,对照组在术后进行常规康复治疗,观察组在术后实施常规康复治疗的基础上行早期系统康复治疗,包括心理疏导、康复宣教、早期消肿、肌力训练、关节活动度锻炼及负重练习,比较两组患者术后膝关节Hohl评分及疗效改善情况。结果随访78例,每组各39例。两组患者治疗后、康复治疗前膝关节Hohl评分比较,差异无统计学意义(P>0.05)。两组患者康复治疗12个月后,观察组Hohl评分显著高于对照组,差异有统计学意义(P<0.05);观察组康复治疗后优良率明显高于对照组,差异有统计学意义(P<0.05)。结论早期系统功能康复治疗明显改善了胫骨平台Schatzker Ⅳ~Ⅵ型骨折环形外固定术后患者膝关节功能,对环形外固定术后的康复起积极作用。Objective To explore influence of early synthetic rehabilitation treatment on knee-joint function of patients with tibial plateau Schatzker type Ⅳ-Ⅵ fractures after circular external fixation. Methods From July 2013 to July 2015, in Harrison International Peace Hospital, 80 patients with tibial plateau Schatzker type Ⅳ-Ⅵ underwent circular external fixation were selected, according to random number table method, they were divided into observation group and control group with 40 cases in each group, the control group was given routine rehabilitation treatment after operation, the observation group was given the early synthetic rehabilitation treatment including psychological counseling, rehabilitation education, early swelling, muscle training, joint activity training, joint loading exercise on the basis of the conventional routine rehabilitation treatment. The Hohl score of knee joint and improvement of curative effect of two groups were compared. Results After operation, 78 pateints were follow-up, 39 cases in each group. After treatment and before rehabilitation treatment, the knee joint Hohl score of two groups were compared, the difference was not statistically significant （P 〉 0.05）. After rehabilitation treatment 12 months, Hohl score in observation group was higher than that in control group, the difference was statistically significant （P 〈 0.05）; after rehabilitation treatment, the excellent and good rate in observation group was higher than that in control group, the difference was statistically significant （P 〈 0.05）. Conclusion Early synthetic rehabilitation treatment of tibial plateau Schatzker type 1V-VI fractures after circular external fixation can improve the knee joint function, and it plays an active role in the rehabilitation of the patients after circular external fixation.
