利多卡因对老年患者认知功能的影响

摘　　要：目的观察利多卡因对老年全麻患者术后认知功能的影响。方法将50例ASAI-Ⅱ级择期全麻下行膀胱肿瘤电切手术的老年患者（60～75岁）随机分为两组：生理盐水组（S组，n=25），利多卡因组（L组，n=25）。两组均以芬太尼、丙泊酚、顺苯磺酸阿曲库胺诱导，并以七氟醚、瑞芬太尼维持麻醉。L组患者在麻醉诱导后予以0．5mg／kg利多卡因复合量缓慢静脉推注，后以0．5mg·kg-1· h-1利多卡因持续泵注至手术结束。S组患者予以等量生理盐水。术中监测心率、血压、脉氧、呼末二氧化碳。记录患者术前、术后6h、术后3d的简易智力状态检查（mini—mental state examination，MMSE）评分。结果两组患者一般资料、麻醉时间等差异均无统计学意义（P〉0．05）；两组患者术后6h及3d与术前比较MMSE评分差异无统计学意义（P〉0．05）。结论利多卡因对老年全麻患者短期术后认知功能无显著保护作用。Objective To observe the effect of lidocaine on cognitive function in aged patients undergoing general anesthesia. Methods All 50 ASA Ⅰ -Ⅱ aged patients （60 ～ 75 years old） undergoing transurethral resection of bladder tumor were randomly divided into two groups, saline group （group S, n=25）, lidocaine group （group L, n=25）. All patients were induced and maintained by fentanyl, sevoflurane and remifentanil. After induced, group L were given lidocaine 0.5 mg/kg bolus followed by 0.5 mg· kg-1· h-1 infusion during operation. Group S were given saline as placebo. The parameters （heart rate, noninvasive blood pressure, pulse oximetry, end-tidal CO2） were recorded, MMSE were performed before anesthesia, and 6 hours, 3 days after operation. Results There were no significant differences between the two groups in age, weight, sex ratio and duration of anesthesia （P 〉 0.05）. No significant difference was found in MMSE score between the two groups on preoperative and postoperative 6 hours or 3 days （P 〉 0.05）. Conclusion There is no significant protective effect on postoperative cognitive function of atomization inhalation of lidocaine.
