摘 要:目的探讨侧脑室肿瘤的CT、MR检查的影像学表现及特点,以提高诊断的准确性。方法回顾性分析经手术病理证实的46例侧脑室肿瘤患者的CT、MR扫描资料,总结影像特征并与病理进行对照。结果46例患者中,室管膜瘤5例,星形细胞瘤8例,胶质母细胞瘤2例,中枢神经细胞瘤4例,脑膜瘤7例,转移瘤4例,淋巴瘤4例,海绵状血管瘤7例,血管周细胞瘤1例,室管膜下瘤2例,室管膜下巨细胞星形细胞瘤1例,神经上皮囊肿1例。中枢神经细胞瘤发病年龄较小,转移瘤发病年龄相对较大,室管膜下瘤、室管膜下巨细胞星形细胞瘤、中枢神经细胞瘤常见于室间孔周围及前角,脑膜瘤、神经上皮囊肿多见于三角区,脑膜瘤、神经上皮囊肿、淋巴瘤密度、信号相对均匀,星形细胞瘤、胶质母细胞瘤、中枢神经细胞瘤、室管膜瘤、血管周细胞瘤囊变常见。结论侧脑室肿瘤的影像学表现有一定的特征,诊断需根据发病年龄、部位及影像特点分析,结合多种影像学检查有助于临床诊断及鉴别。Objective To investigate CT and MRI characteristics of lateral ventricular tumors to improve the diagnostic accuracy. Methods CT and MRI findings were analyzed retrospectively in 46 cases of tumors in the lateral ventricle proved by surgery and pathology, imaging features were concluded in comparison with the pathological findings. Results Ependymoma (n=5), astrocytoma (n=8), glioblastoma (n=2), central neurocytoma (n=4), meningioma (n=7), metastatic tumor (n=4), lymphoma (n=4), cavernous hemangioma (n=7), hemangiopericytoma (n=1), subependymoma (n=2), subependymal giant cell astrocytoma (n=1) and neuroepithelial cyst (n=1) were included in 46 patients. Central neurocytoma was mostly found in young men compared to metastatic tumor. Predilection site of subependymoma, subependymal giant cell astrocytoma, central neurocytoma was comu anterius and interventricular foramen in common, while meningioma and neuroepithelial cyst were located in ventricular trigone. The density and signal of meningioma, neuroepithelial cyst, lymphoma were relatively homogeneous. Cystic area were showed in most astrocytoma, glioblastoma, central neurocytoma, ependymoma and hemangiopericytoma. Conclusion Lateral ventricular tumors have some characteristics using CT and MRI, combining image features with the patient's age and the specific tumor location would contribute to the diagnosis and differential diagnosis of lateral ventricular tumors.
