双J管在上尿路结石合并输尿管狭窄患者中的应用

摘　　要：目的评估留置双J管处理上尿路结石合并输尿管狭窄患者的临床疗效。方法回顾2012年5月至2015年1月45例留置双J管处理上尿路结石合并输尿管狭窄患者的临床资料，所有患者均在行输尿管镜检查时发现有输尿管狭窄，无法行输尿管碎石术，予以留置双J管结束手术，3～6周后拔除双J管，行输尿管镜碎石术。结果43例患者双J管直接置入成功。双J管留置时间为3-6周，平均4．2周，所有病例置管后见输尿管狭窄明显好转，并成功行输尿管软镜碎石术。2例因输尿管狭窄过于严重，无法置八双J管，行经皮肾镜术。结论对于上尿路结石合并输尿管狭窄患者，可先留置双J管，待3-6周后卖施输尿管镜碎石会相对容易和安全。Objective To evaluate the application of double-J ureteral stent in treatment of upper urinary calculi combined with ureterostenosis. Methods From May 2012 to January 2015, 45 patients with upper urinary calculi complicated with ureterostenosis were found under ureterscopy, an F6 double-J ureteral stent was put in ureter, after 3-6 weeks, flexible ureteroscopy lithotripsy was perfromed to eliminate calculi. Results 43 patients insert double-J stent successfully, the ureterostenosis had remission obviously and flexible ureteroscopy lithotripsy were performed in all 43 patients＋ Only 2 patients couldn＇t inserted double-J stent and underwent laparoscopic ureterolithotomy. Conclusion For patients with upper urinary calculi comolicated with ureterostenosis, we can left double-J stent for 3-6 weeks, and then ureteroscopy lithotripsy can be performed easily and safely.
