VSD臀大肌下部肌皮瓣转移术微孔喷气式气垫综合治疗复发型压疮

摘　　要：目的观察负压封闭引流（VSD）、臀大肌下部肌皮瓣转移术及微孔喷气式气垫治疗复发型压疮的临床疗效。方法2010年1月至2015年1月对53例多次术后复发的臀骶部压疮患者，清创后应用VSD，联合臀大肌下部肌皮瓣转移术、微孔喷气式气垫进行治疗。结果本组53例皮瓣全部存活，1例边缘部分坏死，经过植皮后愈合，术后要求患者坚持使用喷气式微孔防压疮气垫。随访期间6个月-2年，未再次发生压疮，已治愈。结论通过清创后应用VSD使创面达到完全彻底的清创效果，再结合臀大肌下部肌皮瓣转移术及术后微孔喷气式气垫综合治疗复发型压疮，达到临床一期愈合，其疗效明确，值得推广。Objective To observe the clinical effect ofVSD, glutes lower myocutaneous flap transfer operation and micropore jet air-cushion on relapsing decubital ulcer. Methods From January 2010 to January 2015, 53 patients with relapsing decubital ulcer underwent VSD combined with glutes lower myocutaneous flap transfer operation and micropore jet air-cushion after debridement. Results Myocutaneous flaps among 53 cases all survived. 1 case complicated with marginal necrosis healed after skin grafting, patients were required to stick with the micropore jet air-cushion postoperatively to prevent decubital ulcer. There had been cured and no recrudescent happened during the follow-up of 6 months to 2 years. Conclusion Modality therapy of relapsing decubital ulcer through the application of VSD after debridement achieve a completely debridement ofthe wound, combined with the glutes lower myocutaneous flap transfer operation and postoperative micropore jet air-cushion to achieve clinical primary healing.Its curative effect is significant and worthy of promotion.
