摘  要:目的:探讨椎体成形术在绵羊骨质疏松脱矿化椎体模型中的生物力学性能变化及可行性.方法:选取绵羊椎体标本30个,随机分为3组,其中A组为未脱钙组,空白对照组;B组为4 h脱钙后椎体成形组;C组为4 h脱钙后假手术组.B、C两组运用微量注射泵向椎体内灌注盐酸及椎体整体浸泡的脱钙方法,快速建立绵羊腰椎骨质疏松脱矿化椎体模型,10个椎体/组.通过骨密度、影像学及生物力学检测,评估椎体成形术在模型中应用的可行性及力学性能变化.结果:BMD检测提示,B、C组模型经脱钙处理后,BMD下降范围在25%~35%,两组比较,差异无统计学意义(P>0.05);与A组比较,差异有统计学意义(P<0.05).X线及CT检查提示椎体成形术在体外模型上具有可操作性,操作程序、骨水泥填充情况与体内及临床试验基本一致.力学实验提示椎体成形术可显著提高椎体的最大抗压强度,各组间比较,差异有统计学意义(P<0.05).结论:绵羊椎体骨质疏松脱矿化模型可以用于椎体成形术的实验研究,并为进一步研究提供理论依据.AIM: To investigate the biomechanical properties and feasibility of vertebroplasty in the demineralized osteoporotic vertebra model of sheep in vitro. METHODS: A total of 30 sheep vetebra model were randomly divided into three groups: group A, undecalcified, blank control group; group B, vertebroplasted group after 4 hours' decalcification; group C, sham operation group after 4 hours' decalcification. Group B and C were treated with the demineralizing method of vertebral perfusion with hydro- chloric acid with micro syringe pump and soaking of vertebra in order to quickly establish demineralized osteoporotic vertebra model of sheep, with 10 vertebrae in each group. The feasibility and the mechanical properties of vertebroplasty were evaluated by bone mineral density, radiographs and biomechanical tests. RESULTS: BMD test showed that BMD was decreased in 25% ~ 35% of group B and C after decalcification. There was no statistically significant difference between group B and group C (P〉0.05), while there were statistically significant differences among group A, B and C (P〈 0.05). X-ray and CT tests showed that vertebroplasty was feasible in vitro model, which remained in line with in vivo and clinical test in procedure and bone cement filling. The biomechan- ical test suggested that vertebroplasty could significantly improve the maximum compressive strength of the vertebrae of group B, and there was statistically significant difference among the threegroups (P〈O.05). CONCLUSION : The demineralized vertebra model of sheep in vitro can be used in the research of vertebro- plasty, which can provide some theoretical basis in the further study.
