静脉用艾司洛尔治疗快速性室上性心律失常的临床研究

摘　　要：目的通过对艾司洛尔注射液在快速性室上性心律失常治疗中的疗效和安全性进行观察研究,进一步为快速性室上性心律失常提供快速、有效的治疗方法。方法选择于我院治疗的快速性室上性心律失常住院患者72例,静脉给予艾司洛尔注射液治疗,观察并记录给药前及给药后30 min患者的心室率、血压及不良反应发生情况。结果与用药前相比,接受静脉用艾司洛尔注射液治疗后患者的心室率均显著降低(P<0.01),但患者的血压与治疗前相比差异无统计学意义(P>0.05);用药后,房颤患者的治疗有效率为83.87%(26/31),房扑患者的治疗有效率为76.47%(13/17),窦速患者的治疗有效率为87.50%(21/24),平均治疗总有效率达83.33%;治疗中共有7例患者出现轻度不良反应,总的不良反应发生率为9.72%,减少药物剂量后不良反应均消失。结论在快速性室上性心率失常治疗中,静脉使用艾司洛尔注射液可快速、有效地转复心律,控制心室率,不良反应少。Objective Through observing the treatment and safety effects of esmolol injection on rapid superventricular arrhythmia, to provide a rapid and effective way for rapid superventricular arrhythmias treatment. Methods Seventy-two cases of patients with rapid superventricular arrhythmias were selected and given esmolol injection. The ventricular rate, blood pressure and side-effection before and 30 minutes after treatment were observed and recorded. Results Compared with that before the treatment, the ventricular rate in patients treated with intravenous esmolol was significantly reduced （P〈0.01）, but there was no significant difference before and after the treatment （P〉0.05）; the effective rates of patients with atrial fibrillation, atrial flutter and nodal tachycardia were 83.87%（26/31）, 76.47% （13/17） and 87.50% （21/24） respectively, and the total effective rate was 83.33%; there were a total of 7 patients with mild adverse reactions, the total adverse reaction rate was 9.72% and the adverse reactions disappeared after reducing the dose. Conclusion In the rapid superventricular atrial arrhythmia treatment, intravenous esmolol injection can quickly and effectively turn the rhythm, control ventricular rate and reduce the adverse reactions.
