摘 要:目的探讨血浆白细胞分化抗原可溶性CD40配体(sCD40L)、可溶性细胞间粘附因子-1(sICAM-1)及血管内皮损伤相关细胞与子痫前期严重程度的相关性。方法选择年龄、BMI、检测孕周等临床一般资料相匹配的重度子痫前期、轻度子痫前期患者以及健康孕妇(对照组)各45例作为研究对象,采用ELISA法检测血浆sCD40L、sICAM-1水平,采用流式细胞仪检测内皮祖细胞(EPC)、血管生成性T细胞(Tang)含量,比较并分析其相关性。结果子痫前期组的血浆sICAM-1、sCD40L水平均明显高于对照组(P<0.05),同时重度子痫前期组上述指标明显高于轻度子痫前期组(P<0.05);重度子痫前期组及轻度子痫前期组EPC、Tang含量均明显低于对照组(P<0.05),重度子痫前期组和轻度子痫前期组上述指标相比较均无明显差异(P>0.05);健康对照组孕妇血浆EPC细胞含量与Tang细胞含量、血浆sICAM-1水平与sCD40L水平均无明显相关性;重度子痫前期组、轻度子痫前期组孕妇血浆EPC细胞含量和Tang细胞含量呈正相关(r=0.654,P<0.05),且两组孕妇血浆sICAM-1水平与sCD40L水平也呈正相关(r=0.624;P<0.05)。结论子痫前期孕妇外周血EPC和Tang细胞含量明显降低,而血浆sICAM-1、sCD40L水平显著升高,二者可能通过诱发内皮细胞损伤进而影响机体免疫和炎症反应参与子痫前期的发病过程。血浆sCD40L、sICAM-1及血管内皮损伤相关细胞水平变化与子痫前期严重程度密切相关。Objective To explore the correlation between sCD40L,slCAM-1,EPC and angiogenic T cells with the severity of preeclampsia. Methods 45 patients diagnosed with severe preeclampsia( SPE group) ,45 patients diagnosed with mild preeclampsia (MPE group) and 45 healthy pregnant women(Control group) of similar age,BMI and gestational age were selected as the research object. The level of slCAM-1 and sCD40L in the three groups were determinated by ELISA,and the percentage of EPC and Tang were determinated by flow cytometry,then analysed the correlation between sCD40L,slCAM-1, EPC and angiogenic T cells. Results The level of slCAM-1 and sCD40L in preeclampsia group was significantly higher than control group, the above indexes in the SPE group was significantly higher than those in MPE group (P〈0.05). The percentage of EPC and Tang cells in MPE group and SPE group were significantly lower than those in the control group (P〈0.05), but no difference was found on the above indexes between MPE group and SPE group (P〉. 05). There was no correlation between the slCAM-1 level and sCD40L level, the percentage of EPC and Tang cells in the control group (P〉. 05). There was positive correlation between the percentage of EPC and Tang cells in the SPE group and MPE group (P〈0.05). And the positive correlation between slCAM-1level and sCD40L level were proved respectively in the SPE group and MPE group (P〈0.05). Conclusion The number of EPC and Tang had decreased significantly, while the level of slCAM-1 and sCD40L had increased,it might induce endothelial injury to affects the immune and inflammatory responses in the pathogenesis of preeclampsia. The level of slCAM-1 and sCD40L,the percentage of Tang and EPC had some correlation with the severity of preeclampsia.
