摘 要:目的比较在临床初诊为2型糖尿病(T2DM)且甲状腺功能正常患者甲状腺相关抗体阳性与阴性的临床特点。方法选取2014年3月至2016年4月在该科住院的初诊T2DM患者413例,测量甲状腺功能及相关抗体的检出率,排除其中75例甲状腺功能异常患者,剩余338例患者分为A组(甲状腺相关抗体阳性组,60例)和B组(甲状腺相关抗体阴性组,278例),比较两组患者年龄、病程、性别构成比、血糖、体质量指数(BMI)、C肽、糖化血红蛋白、尿微量清蛋白及血脂指标。结果两组患者年龄、病程比较,差异均无统计学意义(P>0.05);B组女性患者高于A组,且男性高于女性,差异均有统计学意义(P<0.05)。两组患者BMI、空腹C肽、糖化血红蛋白水平比较,差异均无统计学意义(P>0.05)。但抗体A组的空腹血糖及餐后2小时血糖(2 h PG)高于B组,差异均有统计学意义(P<0.05)。A组患者三酰甘油水平低于B组水平高密度脂蛋白水平高于B组,差异均有统计学意义(P<0.05)。结论甲状腺抗体阳性患者治疗前有更高的血糖水平,但三酰甘油较低。对初诊2型糖尿病患者常规进行甲状腺功能及甲状腺相关抗体检查,可早期发现无症状的甲状腺功能异常及抗体的异常,对临床诊断具有指导意义。Objective To compare the clinical features of the newly diagnosed type 2 diabetes(with normal thyroid function) with positive or negative thyroid antibodies.Method 413 newly diagnosed type 2 diabetic patients from March 2014 to April 2016 were randomly selected.The thyroid function and the related antibodies were measured.75 patients with abnormal thyroid function were excluded.The remaining 338 patients were divided into antibody-positive group(Group A) and antibodynegative group(Group B),and the age,course,gender ratio,blood glucose,BMI,C-peptide,glycosylated hemoglobin,urine trace albumin and blood lipids of both groups were compared.Result There were no significant difference of age,course,BMI,fasting C-peptide and Hb A1 c between those two groups(P〉0.05).However,the group A had notably increased FPG and 2h PG as compared to that of group B(P〈0.05).In addition,group A had a significantly lower TG level and higher HDL level than group B(P〈0.05).Conclusions Patients with positive thyroid antibody had higher blood sugar level and lower TG level before treatment.We sug-gest that the type 2diabetes patients examine the thyroid function and antibody for routine.Early detection of asymptomatic thyroid dysfunction and abnormal antibodies shed light on the clinical diagnosis.
