经口气管插管患者个性化口腔护理综合干预方案的运用效果

摘　　要：目的探讨经口气管插管患者个性化口腔护理综合干预方案的应用效果。方法 2013年8月-2014年9月,便利抽样法选取在第三军医大学新桥医院神经外科经口气管插管行机械通气、入重症监护室(intensive care unit,ICU)时依据改良ICU口腔护理频次评价表评分为6分的患者54例为研究对象。依照入院顺序将其分为对照组和观察组,每组27例。对照组患者按照传统方式进行口腔护理,2次/d。观察组患者采用个性化口腔护理综合干预方案。比较两组患者第2、3、5天首次口腔护理前改良ICU口腔护理频次评分及机械通气第5天时呼吸机相关肺炎(ventilator-associated pneumonia,VAP)的发生率。结果两组患者第2天首次口腔护理前改良ICU口腔护理频次评价的差异无统计学意义(P>0.05),但第3、5天,两组患者首次口腔护理前改良ICU口腔护理频次评价的差异均有统计学意义(均P<0.05)。第5天,对照组有16例患者发生VAP,发生率为59.26%,而观察组仅有8例患者发生VAP,发生率为29.63%,差异有统计学意义(χ~2=4.800,P=0.028)。结论综合口腔护理干预方案有利于保护患者口腔功能,量化口腔环境状态,维持口腔健康状况,减少口咽部细菌滋生,降低VAP的发生率。Objective To discuss application effects of personalized oral care comprehensive intervention program for patients with orotracheal intubation. Methods By convenience sampling, 54 cases with improved oral care frequency evaluation form scored 6 were selected and divided into control group and observation group, with 27 cases in each group.The control group received routine oral nursing, two times a day, while the observation group got personalized oral care comprehensive intervention program. The scores of improved oral care frequency evaluation form in 2nd, 3rd, 5th days and incidence of ventilator- associated pneumonia （VAP） in 5th day of mechanical ventilation were compared between two groups. Results There was no statistical significance of the scores of improved oral care frequency evaluation form in 2nd day was found between two groups（P 〉0.05）, but the statistical significance of the scores of improved oral care frequency evaluation form in 3rd and 5th days were found between two groups（all P〈0.05）.On the 5th day, there were 16 cases had VAP, and the incidence rate was 59.26%, while only 8 cases in observation group, and the incidence rate was 29.63 % （χ2 = 4.800, P = 0.028）.Conclusions Comprehen- sive oral care intervention program can help protecting patients with oral function, quantifying oral environment status, maintaining oral health, reducing oropharyngeal bacteria, reducing the incidence of VAP.
