血液透析患者长期生存率与透析充分性微炎症状态营养状态的相关性分析

摘　　要：目的探究血液透析患者长期生存率与透析充分性、微炎症状态、营养状态的相关性。方法抽取2006年11月~2011年11月来我院就医的150例血液透析患者作为此次实验的目标对象,对150例血液透析患者的临床资料进行回顾性分析,重点对患者的长期生存率、透析充分性、微炎症状态以及营养状态指标进行分析,并比较长期生存率与透析充分性、微炎症状态、营养状态的相关性。结果 150例血液透析患者的1、3、5年生存率分别为85.33%、78.00%、60.67%;150例血液透析患者平均kt/v为(1.35±0.25),患者的上臂皮褶厚度、上臂围放松、上臂围紧张、BMI、白蛋白、血红蛋白分别为(19.45±3.25)mm、(21.34±1.25)cm、(22.19±1.32)cm、(22.42±1.56)kg/m^2、(34.42±2.56)g/L、(114.58±9.32)g/L;CRP、IL-6水平分别为(15.45±5.32)g/L、(85.14±14.25)μg/L;长期生存率和血液透析充分性及营养状态呈现正相关(P<0.05),但长期生存率和微炎症状态呈现负相关(P<0.05)。结论透析充分性、微炎症状态、营养状态对血液透析患者长期生存率均具有一定的影响,为提高患者的远期生存率,临床应结合患者的实际情况对其实施相关的干预措施。Objective To investigate the correlation between long-term survival raterdialysis adequacy,micro inflammation and nutritional status in hemodialysis patients. Methods 150 cases of hemodialysis patients cured in our hospital （November 2006 to November 2011） were selected as the experimental objects, and the clinical data of 150 cases of hemodialysis patients were retrospectively analyzed.The analysis focuses on the long-term survival of patients with hemodialysis, and micro inflammation state and nutritional status index,correlation and comparison the long-term survival rate and dialysis adequacy, nutritional status,inflammation. Results For the 150 cases of hemodialysis patients,the 1,3,5 year survival rates were 85.33%, 78.00%, 60.67%. For the 150 cases of hemodialysis patients, the average kt/v was （1.35 ± 0.25）, and the triceps skinfold, upper arm, upper arm circumference of patients during relaxation of tension, BMI, albumin and hemoglobin were （19.45 ± 3.25）mm,（21.34 ± 1.25）cm （22.19 ±1.32）cm （22.42 ±1.56）kg/m2,（34.42 ±2.56）g/L,（114.58 ± 9.32）g/L.The CRP,IL-6 levels were （15.45 ± 5.32）g/L,（85.14 ±14.25） g/L.The long term survival was positively correlated with blood dialysis adequacy and nutritional status（P 〈 0.05）, but the long-term survival was negative with the microinflammatory state（P 〈 0.05）. Conclusion Hemodialysis, microinflammatory state and nutritional status of hemodialysis patients with long-term survival rate has certain effect. In order to improve the long-term survival of the patients, clinical intervention measures should be combined with the actual situation in the implementation of the.
