摘 要:目的观察比较三种不同面具式敷料对无创通气效果及面部压疮的影响。方法将90例使用无创通气患者按入院先后顺序分为三组,每组30例,分别采用面具式透明敷料外贴、自粘性软聚硅酮泡沫敷料、无粘胶聚氨酯泡沫敷料。观察三组患者的呼吸机漏气量、面部压疮发生率及呼吸相关生理指标的变化。结果使用无粘胶聚氨酯泡沫敷料患者无创机械通气24h漏气量为(516.40±22.00)L/min,面部压疮发生率3.33%,均低于其他两组,差异有统计学意义(P〈0.05);使用前后呼吸频率、心率、PaO2、PaCO2等指标改善程度优于其他两组,差异有统计学意义(P〈0.05)。结论面具式无粘胶聚氨酯泡沫敷料在减少无创机械通气漏气量、降低面部压疮发生率和改善通气效果方面优于面具式透明敷料外贴和自粘性软聚硅酮泡沫敷料。Objective To observe the influence of three kinds of mask dressing on the effect of non- invasive ventilation and facial pressure sores. Methods Totally 90 cases of patients with non-invasive ventilation were divided into three groups according to the order of admission, with 30 cases in each group. Patients in three groups used the mask type transparent external dressing, self adhesive soft silicone foam dressing and non adhesive polyurethane foams dressing respectively. The ventilator air leakage, incidence of facial pressure sores and changes of respiratory related physiological indicators were observed. Results The 24 hour air leakage (516.40±22.00) L/min and incidence of facial pressure sores (3.33%) in non adhesive polyurethane foams dressing group were all lower than the other two groups (P〈0.05). The improvement degree of respiratory rate, heart rate, PaO2, PaCO2 and other indicators in non adhesive polyurethane foams dressing group were all better than the other two groups (P〈0.05). Conclusions The mask type of non adhesive polyurethane foams dressing can significantly reduce the air leakage of non invasive mechanical ventilation and incidence of pressure sores and improve the effect of ventilation obviously.
