未成年恶性骨肿瘤患儿生活质量自评与家长报告差异分析

摘　　要：目的：通过评估和描述未成年恶性骨肿瘤患儿的生活质量状况，比较患儿自评和家长报告结果的差异。方法选择2012年10月─2015年10月在北京协和医院骨科住院的恶性骨肿瘤患儿及其家长共68例，采用PedsQL 4.0儿童生活质量普适性核心量表和PedsQL 3.0儿童癌症模块生活质量量表进行生活质量的自我评估和家长报告。结果恶性骨肿瘤患儿总体生活质量处于较低水平；家长报告在生活质量总体得分上与儿童自评一致（ P〉0.05）。但患儿在“生理”方面以及“疼痛”、“恶心呕吐”、“操作性焦虑”、“治疗性焦虑”及“担忧”上得分高于家长的评分（ P〈0.05）；而在“学校功能”上低于家长报告（ P〈0.05）。结论患儿自评和家长报告对生活质量总体评价，当患儿自评存在困难时，家长报告具有一定的参考价值。但作为独立的个体，患儿和家长所得出的生活质量可能存在部分偏差，因此，在临床工作中应充分结合患儿和家长两者的汇报，以提供更全面、更优质、更个性化的护理。Objective To compare the differences of quality of life （ QOL） between self-rating report and guardian′ report through evaluating and describing the QOL of minors with malignant bone tumors. Methods Totally 68 cases of minors with malignant bone tumors hospitalized in Peking Union Medical College Hospital and their parents were selected from October 2012 to October 2015, and were investigated by PedsQL 4.0 scale and PedsQL 3. 0 scale to collect the QOL of minors evaluated by patients and their parents. Results The overall QOL of minors with malignant bone tumors was at a low level. The results of QOL of parents′evaluation were consistent with the results of self-evaluation of minors （ P〉0.05） . However, the scores of self-evaluation in physiology, pain, nausea and vomiting, operational anxiety, anxiety of treatment and worries were higher than those of results of parents′ evaluation （P〈0.05）, and the score of school function of self-evaluation was lower than that of parents′ evaluation （ P〈0. 05 ） . Conclusions As for the QOL that evaluated by minors and parents, when the minors have difficulties in self-evaluation, the parents′ report has some reference value. As a individual, the QOL from minors themselves and from parents may be different, so in clinical work, we should provide comprehensive personized and high-quality nursing based on the fully combination of minors self-evaluation and parents′report.
