补肾活血法对宫腔粘连术后患者子宫内膜及血流参数影响的临床研究

摘　　要：目的:观察中医补肾活血法对宫腔粘连术后患者子宫内膜及血流参数的影响,探索其作用机理。方法:选择宫腔粘连术后患者80例,随机分成对照组和联合治疗组,各40例。对照组给予雌激素序贯治疗,而联合治疗组在对照组的基础上,加服补肾活血中药汤剂。测定2组患者治疗前后的子宫内膜厚度和血流参数:搏动指数(PI)、阻力指数(RI),观察2组患者的临床效果和3个月宫腔再粘连的发生率。结果:联合治疗组治疗后的子宫内膜厚度比对照组明显增加(P<0.05),PI和RI均较对照组显著降低(P<0.05),其临床总有效率高于对照组(χ~2=4.78,P<0.05)。联合治疗组3个月的再粘连发生率显著低于对照组(7.50%vs 17.50%,P<0.05)。结论:补肾活血中药可改善宫腔粘连术后患者的子宫内膜和血流循环,从而改善临床症状,防止粘连再复发。Objective： To explore the effect and mechanism of the therapy of tonifying kidney and activating blood on endometrium and blood flow parameters after transcervical resection of adhesions（ TCRA）. Methods： 80 patients with intrauterine adhesious after TCRA were randomly divided into control group（ n = 40） and combined treatment group（ n = 40）. Patients in the control group were treated by Sequential Application of Estrogen,and the combined treatment group were based on the control group treated by reinforcing kidney and activating blood cycle therapy for 14 days. Endometrial thickness and Uterine Endometrial Blood Flow（ such as pulsatility index（ PI） and resistance index（ RI）） were measured. The clinical efficacy and the incidence of intrauterine re-adhesions in 3 months were observed. Results： After treatment,the endometrium in the combined group were thicker than the control group（ P〈0. 05）,PI and RI were significantly lower than the control group（ P〈0. 05）. the total clinical efficiency in the combined group were higher（ P〈0. 05）. Furthermore,the incidence of re-adhesion in 3 months were significantly lower than the control group（ 7. 50%vs17. 50%,P〈0. 05%）. Conclusion： The traditional Chinese medicine of tonifying kidney and activating blood can improve the thickness and blood flow of endometrium and the clinical symptoms,prevent the recurrence of adhesion in patients with intrauterine adhesion after TCRA.
