腔内综合技术治疗下肢深静脉血栓形成95例临床分析

摘　　要：目的评价腔内综合技术在治疗下肢深静脉血栓中的临床应用效果。方法回顾性分析2011年6月—2016年7月该院科收治的95例下肢深静脉血栓形成患者行以导管溶栓为主的腔内综合治疗,比较治疗前后双侧下肢周径差,采用静脉通畅评分及静脉通畅率对治疗效果进行评估。结果 95例患者治疗后,症状均得到显著改善。治疗前健患侧大腿周径差(6.83±1.62)cm,治疗后健患侧大腿周径差(1.03±0.93)cm,差异有统计学意义(P<0.01)。治疗前健患侧小腿周径差(4.92±1.06)cm,治疗后健患侧小腿周径差(1.06±0.99)cm,差异有统计学意义(P<0.01)。治疗前静脉通畅评分为(8.40±1.17)分,治疗后为(1.49±0.97)分,差异有统计学意义(P<0.01)。平均静脉通畅率为(88.29±10.38)%。结论以尿激酶加巴曲酶为主要溶栓药物的导管溶栓术在治疗下肢深静脉血栓形成时明显提高溶栓效果,联合下腔静脉滤器植入、一期球囊扩张狭窄、碎栓、术中溶栓、支架植入等腔内综合技术,可以进一步提高疗效、降低风险。Objective To evaluate the clinical application effect of intracavity comprehensive technology in treatment of 95 cases of patients with deep venous thrombosis. Methods 95 cases of patients with deep venous thrombosis admitted and treated in our department from June 2011 to July 2016 were retrospectively analyzed and were treated with intracavity comprehensive technology, and the difference between bilateral lower extremity circumference before and after treatment was compared, and the treatment effect was evaluated by the vein clearance score and vein clearance rate. Results After treatment, the symptoms of 95 cases of patients were obviously improved, and the difference in the healthy and affected side thigh circumference difference before and after treatment had statistical significance [（6.83 ±1.62）cm vs（1.03 ±0.93）cm]（P〈0.01）, and the difference in the vein clearance score before and after treatment had statistical significance, [（8.40 ±1.17）,（4.92 ± 1.06）cm vs（1.06 ±0.99）cm,（8.40 ±1.17） marks vs（1.49±0.97） marks]（P〈0.01）, and the average vein clearance rate was（88.29±10.38）%. Conclusion The catheter directed thrombolysis taking urokinase and batroxobin as the main thrombolysis drugs in treatment of deep venous thrombosis has an obvious effect, which can further improve the curative effect and reduce risks when combined with the intracavity comprehensive technologies such as inferior vena cava filter implantation, first-staged balloon dilatation angioplasty straitness, fragmentation, intraoperative thrombolysis and stent implantation.
