摘  要:目的:探讨基于时间和幅度划分的正电子发射断层(PET)呼吸门控对重建图像伪影的改善效果。方法:采集8例患者的临床数据,比较分析两种呼吸门控方法—基于时间的等时间间隔划分和基于幅度的等数据量划分两种呼吸门控方法;利用最大位移量、最大标准化摄取值(SUV_(max))和肿瘤体积定量研究两种呼吸门控方法;检验SUV_(max)和肿瘤体积利用不同呼吸门控方法是否具有显著差异性。结果:对包含患者心脏部位的数据,基于幅度的方法在最大位移量上比基于时间的方法多检测出24.47%的运动信号。而对肺部病灶,基于幅度的门控方法可以使SUV_(max)恢复到更高的水平(超过基于时间的方法9.92%),其中40%(4/10)的数据差异具有统计学意义(t=-3.805,t=-9.099,t=-2.817,t=-2.315,P<0.05)。在肿瘤体积上,基于幅度的门控方法比基于时间的门控方法减小了29.13%,两者差异有统计学意义(t=2.262,t=10.382,t=2.922,t=2.762,t=2.858,P<0.05),数据比例为50%(5/10)。结论:基于幅度的划分方式可以提取出更多的运动信息,同时保证各时相的图像信噪比相近。Objective: To explore the effectiveness of positron emission tomography(PET) respiratory gating based on time and amplitude to improve the reconstructed image artifacts. Methods: Two respiratory gating methods were compared based on 8 clinical patients data, equal intervals of time division method time-based gating and equal data division method variable amplitude-based gating, the maximum displacement, maximum standardized uptake value(SUVmax) and gross tumor volume were used to quantitatively evaluate these two methods. Independent-t test was used to test whether the difference between two methods were significant for SUVmax and gross tumor volume. Results: For patient data involving heart, amplitude-based method detected 24.47% more motion signal which related to maximum displacement than time-based method. For patient data involving lung lesions, SUVmaxderived from amplitude-based method could be recovered to a higher value(9.92% more than time-based method), and 40%(4/10) of the patient data results were statistically significant(t=-3.805, t=-9.099, t=-2.817, t=-2.315, P〈0.05). Gross tumor volume calculated from amplitude-based method could be 29.13% less than time-based method, and the differences were statistical significant(t=2.262, t=10.382, t=2.922, t=2.762, t=2.858, P〈0.05), besides the percentage of patient data was 50%(5/10). Conclusion: Amplitude-based method can extract more motion information, at the same time, signal to noise ratio values of different phases' images are similar to each other.
