摘  要:目的:探讨桥接组合式内固定系统(BCIFS)应用于四肢和骨盆骨折的临床效果。方法:对95例四肢和骨盆骨折患者资料进行回顾性研究,根据不同手术方法将其分为观察组(48例)与对照组(47例),观察组采用BCIFS手术,对照组采用传统切开复位内固定术;术后随访两组患者骨折复位及功能恢复情况,比较两组患者的手术时间、术中出血情况及术后恢复情况。结果:观察组平均手术时间为115.6 min,术中平均出血量为360.0 ml,骨折愈合平均时间为4.5个月,肢体功能平均评分为83.2分,该4项指标均优于对照组,差异有统计学意义(t=6.831,t=16.741,t=6.304,t=9.246;P<0.05)。术后随访,观察组患者伤口均一期愈合,未发生切口感染等术后并发症,骨折全部愈合,其平均愈合时间为4.5个月,肢体功能评估优良率为93.75%。对照组患者发生切口感染1例,骨折延迟愈合2例。骨折平均愈合时间为6.4个月,肢体功能评估优良率为82.98%。结论:BCIFS手术在四肢和骨盆骨折中具有较好的应用价值,其手术具有操作灵活、固定效果可靠及并发症少等优点。Objective: To investigate the clinical effect of bridge combined internal fixation system(BCIFS) in fractures of limb and pelvis. Methods: 95 patients with fracture of limb and pelvic were studied by using retrospective analysis, and they were divided into observation group(48 cases, treated with BCIFS) and control group(47 cases, treated with traditional open reduction and internal fixation) according to the different surgical methods. After post-operation, followed up these patients for their situations of fracture reduction and function recovery; compared operation time, intra-operative hemorrhage and postoperative recovery situation between two groups. Results: The average operative time was 115.6 min; the average amount of bleeding was 360 ml; the average time of fracture healing was 4.5 months; and the average score of limb function was 83.2 in control group. The four indexes of control group were better than the control group(t=6.831, t=16.741, t=6.304, t=9.246; P〈0.05). Postoperative follow-up showed all of wounds healed primarily without complications, all of fractures has healed, and fineness ratio of limb function was 93.75% in observation group. While 1 patient was found infection of wound; 2 patients were found delay in the healing of fracture; the average time of fracture healing was 6.4 months and fineness ratio of limb function only was 82.98% in control group. Conclusion: BCIFS surgery, with flexible operation, reliable fixation and fewer complications, has better application value in the limbs and pelvic fractures.
