摘 要:目的研究通络方干预大鼠冠状动脉微栓塞的作用及机制。方法将26只雄性SD大鼠分为通络方组(n=10)、模型组(n=10)、空白组(n=6)。经导管介入法主动脉根部注入0.3mL自身血栓颗粒建立冠状动脉微栓塞模型。通络方组采用干预药物灌胃9d,模型组和空白组给予灭菌水灌胃9d。采用ELISA检测法观察血清肌钙蛋白I(cTnI)浓度;左心室心尖部切片病理染色、免疫组化分别观察栓塞的血管数量和缺血心肌面积,心肌肿瘤坏死因子-α(TNF-α)和白细胞介素(IL)-1β水平。结果 HE染色显示:模型组血管栓塞数量高于空白组(P<0.05),造模成功:通络方组血管栓塞数量与模型组比较,差异无统计学意义(P>0.05),但观察到冠脉微血管栓子明显缩小。HBFP染色染示模型组心肌缺血面积明显大于通络方组和空白组,差异有统计学意义(P<0.05)。通络方组血清cTnI较模型组明显降低,差异有统计学意义(P<0.05);通络方组心肌TNF-α和IL-1β明显低于模型组,差异有统计学意义(P<0.05)。结论通络方可抑制冠脉微栓塞病理进程,减少心肌细胞损伤,抑制炎症因子表达,促进微栓塞血管机化再通。Objective To explore the effect and its mechanism of Tongluo decoction(TLD)on the coronary microembolization in rats.Methods Twenty-six male SD rats were divided into TLD group(n =10),model group(n =10),control group(n =6).The rats of TLD group and model group were injected 0.3 mL autologous micro thrombus particles by intervention operation via right common carotid artery for building coronary microembolization model.Postoperation 24 hours,rats of TLD group were given intervention drug by gavage once daily for 9 days.The rats of model group and control group were given sterile water by gavage once daily for 9 days.To observe the number of vascular embolization and the area of myocardial ischemia,taking the apex of left ventricular with paraffin section through pathological staining.The level of serum cTnI was tested by ELISA.The expression of tumor necrosis factor-α(TNF-α)and interleukin(IL)-1βin myocardium were tested by immunohistochemical.Results HE staining showed that the number of coronary microembolization in model group was obviously higher than that in control group(P〈0.05).It showed that coronary microembolization model of rats were built successfully.There was no difference in the number of coronary microembolization between TLD group and model group(P〉0.05).The emboli of coronary microembolization in TLD group were becoming smaller than ever and obviously organized.HBFP staining showed that the positive staining area in model group was significantly bigger than that in TLD group and control group(P〈0.05).Compared with model group,serum cTnI level in TLD group was decreased(P〈0.05).The expression of TNF-αand IL-1βin myocardium in TLD group was obviously lower than that in model group(P〈0.05).Conclusion TLD could significantly inhibit the pathological change of coronary microembolization,reduce the injury of myocardial cell,inhibit the expression of inflammatory cytokines and promote the microemboli vascular recanalization.
