祛腐生肌散促进肛瘘术后创面愈合的临床观察

摘　　要：目的观察祛腐生肌散促进肛瘘术后创面愈合的临床疗效。方法将90例肛瘘术后患者随机分为2组,治疗组45例予祛腐生肌散换药,对照组45例予复合溶葡萄球菌酶杀菌纱布换药。比较2组术后7、14、21 d创面愈合率,记录术后1、3、7、14 d创面分泌物评分,比较2组腐肉脱落时间、创面愈合时间及术后7 d疼痛积分,比较2组术后28 d临床疗效。结果 2组术后14、21 d创面愈合率比较差异均有统计学意义(P<0.05),治疗组高于对照组。2组术后21 d创面愈合率高于本组术后14 d(P<0.05)。2组术后3、7、14 d创面分泌物评分比较差异均有统计学意义(P<0.05),治疗组低于对照组术后同期。2组术后7 d创面分泌物评分均低于本组术后3 d(P<0.05),2组术后14 d创面分泌物评分均低于本组术后7 d(P<0.05)。治疗组腐肉脱落时间、创面愈合时间均较对照组提前(P<0.05),术后7 d疼痛积分低于对照组(P<0.05)。治疗组痊愈率95.56%,对照组80.00%,2组痊愈率比较差异有统计学意义(P<0.05),治疗组术后28 d临床疗效优于对照组。结论祛腐生肌散能有效缓解肛瘘术后创面疼痛,减少创面分泌物,加快创面愈合,安全有效。Objective To observe the clinical effects of Expel-decay and promote tissue regeneration powder on wound healing after operation of perianal abscess. Methods 90 patients after operation of perianal abscess were randomly divided into two groups. Dressing change by of Expel-decay and promote tissue regeneration powder was used in treatment group（n=45）,and dressing change by compound lysostaphin sterile gauze was used in control group（n=45）. The wound healing rats 7,14 and 21 d postoperation were compared between two groups,the wound excretion score 1,7,14 and 21 d postoperation were recorded. The slough away time,wound healing time and pain score of 7 d after operation were compared between two groups. The curative effect was compare between two groups28 d postoperation. Results The wound healing rats 14 and 21 d postoperation in treatment group were higher than those in control group with statistical differences（P〈0. 05）. The wound healing rat of 21 d postoperation was higher than 14 d postoperation in two groups（P〈0. 05）. The wound excretion scores 3,7 and 14 d postoperation in treatment group were lower than those in control group at the same period,with statistical differences（P〈0. 05）. The wound excretion score of 7 postoperation was lower than 3 d postoperation in two groups（P〈0. 05）,and the score of14 d postoperation was lower than 7 d（P〈0. 05）. The slough away time and wound healing time in treatment group were shortened than control group（P〈0. 05）,and the 7 d pain score was lower（P〈0. 05）. The cure rate in treatment and control group were 95. 56% and 80. 00% respectively,with statistical differences（P〈0. 05）. The curative effect 28 d postoperation in treatment group was superior to control group. Conclusion Expel-decay and promote tissue regeneration powder can relieve pain after operation of perianal abscess,reduce wound excretion,enhance wound healing,safe and effective.
