显微喉镜下CO_2激光治疗会厌囊肿的临床研究

摘　　要：目的探讨显微喉镜下CO_2激光切除会厌囊肿的手术方法及疗效。方法回顾分析经CO_2激光治疗的58例会厌囊肿的临床资料。会厌囊肿位于会厌舌面近中线46例、会厌谷7例、杓会厌襞5例。采用CO_2激光切除会厌囊肿,分析手术疗效及相关注意事项。结果手术后所有患者均有轻微咽痛、吞咽痛和不同程度的咽部异物感。术后第2天见原会厌囊肿处有创面及灰白假膜,略肿胀,无活动性出血。术后1个月复查,无咽痛不适,部分患者仍有异物感。58例会厌囊肿,1次治愈55例。术后复发3例(5.2%),均为会厌谷囊肿,由于囊肿底接近或嵌入舌根,再次行囊肿揭盖术,未再复发。结论 CO_2激光治疗会厌囊肿效果好,损伤小,术后无需使用抗生素和激素治疗。会厌谷囊肿如囊肿底近舌根或嵌入舌根的,可采用揭盖术进行治疗。Objective To investigate the surgical procedure and the clinical effects of endoscopic CO2 laser microsurgery on epiglottic cyst. Methods Data of 58 patients with epiglottic cyst treated with CO2 endoscopic laser microsurgery were analyzed respectively. In these cases,46 were located at the lingual surface of the epiglottis,7 at the vallecular and 5 at aryepiglottic fold. Endoscopic CO2 laser microsurgery was performed and the prognosis was analyzed. Results All of the patients had slight angina,odynphagia and foreign body sensation postoperatively.Wound and pseudomenbrane could be seen on the second day. The epiglottis had slight swellen without bleeding. None of the patients complained angina one month later. Seldom patients still complained foreign body sensation of the pharynx. Fifty-five patients were cured. Three patients of vallecular cyst had relapse. Because the bottom of the cyst was close to or in the base of tongue,uncover method was performed and no relapse occured. Conclusions Endoscopic CO2 laser microsurgery is an effective method to treat epiglottic cyst. Antibiotics and glucocorticoids are not needed postoperatively. Uncover method surgery is a good choice for vallecular cyst when the bottom is close to or in the base of tongue.
