CT仿真内窥镜在结肠病变中的应用

摘　　要：为探讨CT仿真内窥镜（CTVE）对结肠癌及息肉病变的诊断价值，利用16排螺旋CT（MSCT）对93例临床诊断为结肠癌和结肠息肉的患者进行容积扫描，再利用设备自带后处理软件对扫描数据进行重建、处理，并将影像诊断结果与电子结肠镜检查结果进行对照分析。结果显示，电子结肠镜检查并经取活检病理诊断结肠癌35例，结肠息肉40例，无异常发现者18例；CTVE诊断结肠癌33例，结肠息肉38例，诊断结肠占位性病变的敏感性为94．7％，特异性83．3％，假阳性率16．7％，假阴性率5．3％，准确率92．5％。CTVE更有利于占位性病变直径〉5mm病变的检出，在诊断〈5mm占位病变时，其假阴性率将增加，需结合其他相关资料进一步诊断。结果表明，在检查前充分的肠道准备的前提下，CTVE的成功率极高。在结肠占位性病变的诊断中具有较高的价值，因无创、无痛苦，无明显禁忌证，将在结肠占位病变的诊断中发挥更大的作用。In order to explore the diagnostic value of CT visual endoscope（CTVE） for colonic carcinoma and polyps lesion by using 16-slice CT,authors took volume screen on the patients with colonic carcinoma and polyps confirmed clinically,then using its fitted post-processing software performed reconstruction and processing on screen-data and analyzed ＆ compared the obtained image diagnostic results with electro-colonoscopy data.As results,through electro-eolonoscopy and biopsy pathological exam., 35 cases were diagnosed as colonic cancer,40 as colonic polyps,the rest 18 as normal;meanwhile in CTVE 33 cases as colonic cancer.38 as colonic polyps,in the diagnosis of located lesions the sensitivity was 94.7%, specificity, 83.3%,false-positive rate 16.7%, false-negative rate 5.30//00, accuracy rate 92.5%; Results showed CTVE was beneficial to detect the located lesion（〉5 mm）,for the lesion（〈5 mm）,its false-negative rate would be increased when need to take further diagnosis combined other relevant materials. Results show that when pre-examinational intestinal preparation was done. The scuccess rate of CTVE just would be very high, it has higher diagnostic value for colonic located lesions and will play more important role due to its noninvasive feature,painless and without significant contraindication.
