摘 要:目的探讨腰围甘油三酯(WT)指数与胸痛患者心血管危险因素聚集的关系。方法回顾性分析在秦皇岛市第一医院就诊的胸痛患者,比较心血管危险因素聚集组和非聚集组基线资料的差异,分析腰WT指数与心血管危险因素聚集的相关性。结果共有309例患者人选,其中女性115例。心血管危险因素聚集的检出率为73.8%。心血管危险因素聚集组的体重、腰围、体重指数、低密度脂蛋白胆固醇、甘油三酯、空腹血糖和WT指数均高于非聚集组(P〈0.05);高密度脂蛋白胆固醇低于非聚集组(P〈O.05)。WT指数与心血管危险因素数量呈正相关(r=0.414,P〈0.01)。多因素Logistic回归表明,WT指数是心血管危险因素聚集的独立预测因素(OR=I.012,95%CI1.008-1.018)。WT指数在全部研究人群、男性人群及女性人群ROC曲线下面积分别为O.750(95%C10.689-0.811)、O.709(95%C10.631-0.788)和0.852(95%C10.771-0.933);其预测心血管危险因素聚集的最佳切点分别为138.7、110.6和134.4。结论WT指数是心血管危险因素聚集的独立预测因素,而且可以对心血管危险因素聚集做出定量评价。Objective To study the relationship between waist circumference-triglyceride(WT) index and clustering of cardiovascular risk factors in chest pain patients. Methods This was a retrospective cohort study. Chest pain patients admitted to our hospital from April 2014 to December 2015 were enrolled. The basic characteristic differences between clustering of cardiovascular risk factors group and no clustering of cardiovascular risk factors group were analyzed. The relationship WT index and clustering of cardiovascular risk factors was analyzed by Pearson relationship analysis and logistic regression analysis. Results A total of 309 patients were enrolled, 115 of them were female. The prevalence of cardiovascular risk factors was 73.8%. Levels of weight, waist circumfer- ence, body mass index, low-density lipoprotein cholesterol, triglyceride, fasting blood glucose and WT index in cardiovascular risk factors group were higher than that in clustering of cardiovascular risk factors group (P〈0.05). The level'of high-density lipoprotein cholesterol in cardiovascular risk factors group was lower(P〈0.05 ). WT index was correlated with number of clustering of cardiovascular risk factors (r=0.414, P〈0.01 ). WT index was an inde- pendent risk factor of clustering of cardiovascular risk factors by multivariate logistic regression analysis. The re- ceiver operating characteristic(ROC ) curves of WT index in total patients, male patients, and female patients were 0.750(95%CI 0.689-0.811), 0.709(95%CI 0.631-0.788), and 0.852(95%CI 0.771-0.933), respectively; and the cutoff values of WT index were 138.7, 110.6, and 134.4, respectively. Conclusion WT index is an indepen-dent risk factor of clustering of cardiovascular risk factors and could assess quantitatively risk factor of clustering of cardiovascular risk factors.
