肩关节周围手术中行超声联合神经刺激器肌间沟臂丛麻醉的效果观察

摘　　要：目的分析超声联合神经刺激器肌间沟臂丛麻醉用于肩关节周围手术的临床效果。方法选取2013年9月-2015年11月山西省闻喜县人民医院收治的62例肩关节周围骨折患者，按数字表法分为对照组和观察组各31人。对照组采用神经刺激器引导肌间沟臂丛麻醉，观察组采用超声联合神经刺激器肌间沟臂丛麻醉。对两组患者的穿刺次数、麻醉操作以及并发症进行对比分析。结果观察组的穿刺次数为（1．6±0．5）次，低于对照组的（3．5±0．8）次，差异有统计学意义（P〈0．05），并发症和治疗效果上观察组也优于对照组。结论超声联合神经刺激器肌间沟臂丛麻醉在肩关节周围手术中应用，具有操作简单、安全性高以及手术痛苦较小等优点，值得临床推广使用。Objective To analyze the effect of interscalene brachial plexus anesthesia with ultrasound combined with nerve stimulator in surgery around shoulder joint. Methods Sixty-two patients with shoulder joint fracture were selected in Wenxi County Hospital of People of Shanxi Province from September, 2013 to November, 2015 and divided into the control group and the observation group according to the number table method with 31 patients in each group. The control group was given nerve stimulator guided interscalene brachial plexus anesthesia, while the observation group was given interscalene brachial plexus anesthesia with ultrasound combined with nerve stimulator. The numbers of puncture, anesthesia procedures and complications of two groups were analyzed. Results The number of puncture in the observation group was （1.6±0.5） times and less than that of the control group which was （3.5±0.8） times, and the difference was statistically significant （P 〈 0.05）. Complications and treatment effect of the observation group were better than those of the control group. Conclusion There are advantages of the application of interscalene brachial plexus anesthesia with ultrasound combined with nerve stimulator to around shoulder joint surgery, such as simple operation, high safety and small operation pain. The method of treatment is worthy of clinical application and promotion.
