肺部超声检查联合血浆降钙素原、C反应蛋白检测对社区获得性肺炎的诊断价值

摘　　要：目的探讨肺部超声检查联合血浆降钙素原(PCT)、CRP检测对社区获得性肺炎(CAP)的诊断价值。方法对临床疑诊CAP患者240例同时进行肺部超声和胸部CT检查,并检测血清PCT、CRP及计数WBC。以胸部CT结果作为诊断CAP"金标准",分析肺部超声及血浆PCT、CRP对CAP的诊断价值。结果肺部超声诊断CAP的特异性为72.73%、敏感性为95.65%。CRP升高诊断CAP的敏感性为97.66%,特异性为30.77%;PCT升高诊断CAP的特异性为96.15%,敏感性为42.52%;WBC数量增加诊断CAP的特异性、敏感性分别为88.46%、84.58%。肺部超声检查联合血浆PCT、CRP检测诊断CAP的,特异性为92.59%,敏感性为99.06%。结论肺部超声检查联合血浆PCT、CRP检测对CAP的诊断敏感性及特异性均优于单项检测。Objective To investigate the diagnostic value of pulmonary ultrasonography （ LUS） combined with plasma calcitonin （ PCT ） and C-reactive protein （ CRP ） in the diagnosis of community acquired pneumonia （ CAP ） . Methods LUS and chest CT were performed in 240 cases of clinically suspected CAP patients at the same time , and we detect the levels of plasma PCT , CRP and WBC count .Taking chest CT results as a diagnostic CAP ＂gold standard＂, we analyzed the diagnostic value of LUS examination , serum PCT and CRP detection in the diagnosis of CAP .Results The specificity and sensitivity of CAP were 72.73%and 95.65%, which were diagnosed by LUS .The sensitivity of increased CRP in the diagnosis of CAP was 97.66%, and the specificity was 30.77%.The specificity of increased PCT in the diag-nosis of CAP was 96.15%, and the sensitivity was 42.52%.The sensitivity and specificity of increased WBC in the diag-nosis of CAP were 88.46%and 84.58%, respectively.The diagnostic sensitivity of LUS combined with PCT , CRP and CAP was 99.06%, and the specificity was 92.59%.Conclusion The sensitivity and specificity of LUS combined with serum CAP and PCT detection in diagnosis of CRP are superior to those of single test .
