摘 要:目的总结近2年潍坊医学院附属医院新生儿科血培养检出菌分布及耐药状况,以指导临床抗菌药物应用。方法选取我院1519例患儿(日龄≤28d)血培养分离出致病菌进行回顾总结及耐药分析。结果检出致病菌共276株(18.2%),分别含革兰阳性菌222株、革兰阴性菌45株及真菌9株;革兰阳性菌的首位检出菌为凝固酶阴性葡萄球菌(CNS),其次为金黄色葡萄球菌,对青霉素类、头孢类药物高水平耐药,未检出万古霉素耐药株(VRSA)及替考拉宁耐药株;革兰阴性菌前两位检出菌为肺炎克雷伯菌(KP)及大肠埃希菌(E.coli),对碳青霉烯类抗生素敏感,对常用抗生素耐药率高。结论我院新生儿科血培养以革兰阳性球菌检出为主,种类繁多且多重耐药,临床应及早送检血培养以指导治疗用药。Objective To analysis the pathogenic bacteria distribution and drug resistance of cultivated strains of the blood culture form recently two years in Neonatology Department, to guide clinical use of antimicrobial. Methods A total of 1519 cases of children (ages ≤28d) isolated from the blood culture pathogenic bacteria and drug resistance were retrospectively analyzed and summarized. Results Two hundred and seventy-six cases of blood samples were positive, the positive rate was 18.2%. The amount of gram-positive bacteria gram-negative bacteria and fungus were 222,45 and 9. Ranking first in Gram-positive bacteriais CNS, and followed by staphylococcus aureus, showed highly re- sistant to penicillin,but sensitive to vancomycin. Ranking first and second in Gram negative bacteria were KP and E. co- //,both showed sensitive to Carbapenems, but highly resists to the commonly used antibiotics. Conclusion The chief bacteria of cultivated strainsis Gram-positive coccus,which are wide variety and multi drug resistance. In order to effica- ciously guide the early treatment, the blood culture sample should be send as early as possible.
