TACE联合碘125粒子与联合射频消融治疗复发性肝癌临床观察

摘　　要：目的对比分析经皮肝动脉化疗栓塞术(TACE)联合碘125粒子植入及TACE联合射频消融治疗复发性肝癌的疗效及安全性。方法回顾性分析2013年1月~2015年6月我院诊治的90例复发性肝癌患者的临床资料,其中行TACE治疗(对照组)40例,采用TACE联合碘125粒子植入治疗30例,采用TACE联合射频消融治疗20例,观察3组患者治疗后甲胎蛋白(AFP)和谷丙转氨酶(ALT)变化、术中及术后不良反应以及治疗后12个月、18个月生存率。结果治疗后TACE联合射频消融组患者AFP下降较其他两组显著,生存率高于其他两组,组间比较差异均有统计学意义(P<0.05)。结论两种联合治疗较TACE治疗均能提高复发性肝癌患者生存率,降低患者AFP水平;联合射频消融组患者生存率高于联合碘125组。Objective To compare the clinical effect and the safety of TACE combined with iodine 125 seed anti TACE com- bined with radiofrequency ablation in treating recurrent hepaticelluar carcinoma. Methods The clinical data of 90 patients with recurrent hepatocellular carcinoma treated in our hospital from Janum7 2013 to June 2015 were retrospectively analyzed. Forty of them were treated by TACE,as the contrast group. Thirty of them were lreated by TACE combined with iodine 125 seed,20 of them were treated by TACE com- bined with radiofrequency ablation. Obseved the A FP and ALT variation after treatment,untoward effect during and after the operation,survive rate of 12 months and 18 months after treatment. Results AFP of patients treated by TACE combined with radiofrequency ablation declined more significantly,the survive rate was also higher than the other two groups,the differences between groups had statistical significance（ P 〈 0.05 ）. When accepted lhe radiofrequency ablation treatment,some patients t^lt liver area pain. Conclusion Both of the two combined treatment can increase the survive rate of recurrent hepatoeellular carcinoma patients, also decrease the AFP level. The survive rate of patients treated by TACE combined with radiofrequency ablation is higher than patients treated by iodine 125 seed.
