医院倒班制临床从业群体职业应激现况及对心血管危险因素的影响分析

摘　　要：目的分析医院倒班制临床从业群体职业应激现况及对各类心血管危险因素影响。方法选择2016年3~5月本院体检的临床从业医护人员体检大数据作为分析资料来源,其中97例为倒班制医护从业人员(临床倒班制组),另择本院104例白班制医护从业人员为对照组,两组入选对象体检期间均接受"职业紧张调查表(OSI-R)修订版"问卷调查。结果 OSI-R问卷调查数据临床倒班制组显著高于对照组(P均<0.05)。血压及心率测量数据中,临床倒班制组的平均收缩压、24小时平均收缩压及24小时平均心率均显著高于对照组(P均<0.05)。血生化检测数据中,临床倒班制组的餐后2小时血糖及血总胆固醇浓度均显著高于对照组(P均<0.05)。结论医院临床倒班制从业群体职业应激水平程度重,存在着血压、心率、血糖和血胆固醇增高表现。Objective To analyze the influence of night shifts on occupational stress and various cardiovascular risk factors in medical staff. Method The data was obtained from the physical examination of the medical staff in Liaoning Provincial People＇s Hospital in 2016. They were selected as the data source for the study of occupational stress and cardiovascular risk factors. Among them, 97 cases were on night shifts, and 104 cases were on regular shifts as the control group. Both of them accepted the Occupational Stress Inventory Revised Edition（OSI-R） survey. Result In the OSI-R questionnaire, the scores of ORQ and PRQ in the night shifts group were significantly higher than those in the control group（P〈0.05）. The average systolic blood pressure, 24 h-mean systolic blood pressure and 24h-average heart rate were significantly higher in the night shifts group than in the control group（P〈0.05）. And so as the blood biochemical test data, 2 h postprandial blood glucose and total cholesterol were significantly increased in the night shifts group than control group（P〈0.05）. Conclusion The occupational stress level of medical staff on night shifts is higher than the regular shifts group. The medical staff on night shifts has much cardiovascular risk factors such as hypertension, tachycardia, hyperglycemia and hyperlipidemia. Thus, they are susceptible to cardiac disease.
