摘 要:研究目的:评估急性期脑梗死患者运动功能障碍患者的脑区时间变异性改变及其临床预后相关性;材料与方法:收集20位急性脑梗死患者梗死后1周及2周内的两次静息态功能磁共振数据以及患者三个月后的运动功能评估量表,运用复旦冯建峰团队的时间变异性程序计算脑区的时间变异性,并采用配对T检验比较患者状态的改变,采用Pearson相关评价时间变异性改变在临床运动评估的能力。结果:对比第一次扫描,脑梗死患者发生改变的脑区主要在运动脑区,额叶脑区及皮层下脑区,患侧中央前回的变异性改变程度与临床量表的改变程度正相关。结论:功能磁共振及时间变异性指标是有效的评价卒中患者运动功能改变及恢复的工具,皮下脑区的改变可能与代偿机制有关。Objective:To investigate changes of regional temporal variability for acute stroke patients with hemiparesis.Material and Method:A total of 20 patients underwent MRI examinations at two consecutive time points after the first ever ischemic stroke(one week and two week after stroke.At each time point,a rest state f MRI(RS-f MRI) scans was performed.The hand motor function of each patient was assessed using Fugl-Meyer Motor Assessment(FMA) at admit and three months later.The time variability program,produced by Prof.Feng Jianfeng of Fudan University,was used to calculate the index of RSf MRI.Paired T test was used to assess Changes of regional temporal variability for acute stroke patients with hemiparesis between two time points.Correlations between the increment value of variability and motor impairment score was analysed by Pearson correlation tests.Results:Paired T test showed that these regions,mainly located at sensory-motor areas,frontal lobe area and subcortical area.Correlation analyses between variability change and FMA change revealed that the increment value of variability in ipsi-lesional precentral gyrus was positively correlated to the increment value of motor score in the three month after stroke.Conclusion:RS-f MRI and the time-variability index could be used to evaluate the brain changes of stroke patients and might be used to be the marker of recovery of motor ability.The changes of subcortical area might be related to the mechanism of compensatory.
