掌背穿支皮瓣滑移术在先天性并指畸形指蹼重建中的应用

摘　　要：目的研究在先天性并指畸形指蹼重建过程中掌背穿支皮瓣滑移术的应用效果。方法选取我院收治的80例先天性并指畸形病例纳为研究对象,均于指蹼重建中实施掌背穿支皮瓣滑移术,观察术后皮瓣成活情况与面积、指蹼外形、指蹼两点辨距觉、手功能及患儿家长满意度。结果 80例126个皮瓣均成活,成活率100.00%,皮瓣最大面积与最小面积分别为3.14cm×2.05cm、1.47cm×1.03cm,指蹼外形与掌指关节活动均恢复正常,指蹼两点辨距觉为(11.24±1.06)mm;手功能优良率达到94.44%,且家长总满意率高达97.50%。结论对先天性并指畸形患者指蹼重建实施掌背穿支皮瓣滑移术,能保证重建指蹼外形具有美观性,有效恢复手功能,安全高效,推广价值高。Objective To study the application effect of the dorsal metacarpal perforator sliding flap for web-space reconstruction in congenital syndactyly. Methods A total of 80 cases of congenital syndactyly in our hospital were enrolled in the study. The dorsal metacarpal perforator sliding flap was applied in web-space reconstruction. The postoperative survival and area of flaps,shape of fingerweb,two-point discrimination,hand function and parents＇ satisfaction were observed. Results In 80 cases,126 flaps survived,and the survival rate was 100.00%. The largest and smallest areas of flaps were 3.14cm×2.05cm、1.47cm×1.03 cm,respectively. The shape of fingerweb and metacarpophalangeal joint activities were normal,and the two-point discrimination was（11.24±1.06） mm. The excellent and good rate of hand function was 94.44%,and the total satisfaction rate of parents was 97.50%. Conclusion The application of dorsal metacarpal perforator sliding flap for web-space reconstruction in patients with congenital syndactyly can guarantee the appearance of fingerweb and effectively restore the hand function,and it is safe.
