负压封闭辅助技术在修复皮瓣无法覆盖大面积创面中应用体会

摘　　要：目的探讨负压辅助封闭技术(VAC)在修复体表肿物切除术后,无法利用皮瓣覆盖的大面积创面中的作用。方法选择2015年1月至2016年8月中国医科大学附属第一医院收治的5例巨大体表肿物患者为研究对象。本组患者均一期行肿瘤切除术后给予VAC治疗,二期行自体皮片游离移植术。结果所有患者经过2～3次的VAC治疗后,创面新鲜,植皮后皮片成活良好,外形无明显凹陷,无功能障碍,患者对术后效果满意。结论巨大体表肿物切除术后创面无法行皮瓣手术予以修复,利用VAC装置促进创面生长,配合自体皮片游离移植术可取得良好的治疗效果。Objective To investigate the effect of vacuum assisted closure（VAC） in body surface neoplasm resection postoperative wound repair,where cannot use flap coverage. Methods A retrospective study was performed on 5 patients with large body surface mass who were admitted from January 2015 to August 2016. Patients underwent VAC therapy after tumorectomy in phase I,autologous skin grafting in phase II. Results After 2-3 times of VAC therapy in all patients,the wound was fresh,skin graft survived well after grafted,there was no obvious sunken on appearance,no dysfunction,and patients were satisfied with the postoperative effect. Conclusion VAC device promotes wound growth and assists autologous skin grafting to achieve a good therapeutic effect on large surface after neoplasm resection,where cannot be performed flap repair surgery.
