术前中药处理在老年结石性胆囊炎行腹腔镜胆囊切除术中的效果分析

摘　　要：目的观察术前中药处理在结石性胆囊炎行腹腔镜胆囊切除术中的应用效果。方法将80例结石性胆囊炎患者随机分为对照组和观察组,每组40例。对照组行常规术前准备,观察组术前给予中药处理(苁蓉承气汤)。两组均行腹腔镜胆囊切除术治疗,比较两组治疗效果、36 h内恶心呕吐发生情况、手术时间、术中出血量、住院时间、术后排气时间及肠鸣音恢复时间。结果观察组术后36 h内恶心呕吐Ⅰ级比例显著高于对照组,Ⅱ级少于对照组(P<0.05),手术时间、术中出血量、住院时间均显著优于对照组(P<0.05);观察组治疗总有效率显著高于对照组(P<0.05);术后排气时间、肠鸣音恢复时间显著短于对照组(P<0.05)。结论通过术前给予中药苁蓉承气汤可显著提高腹腔镜胆囊切除术的临床治疗效果,值得推广应用。Objective To observe the application effect of preoperative Chinese medicine treatment on laparoscopic cholecystectomy in patients with calculous cholecystitis. Methods Eighty patients with calculous cholecystitis were randomly divided into control group and observation group, with 40 cases in each group. The control group given routine preoperative preparation, the observation group received the preoperative Chinese medicine treatment （Congrong tongbian soup）, the two groups were performed laparoscopic cholecystectomy. The treatment effect, the occurrence of nausea and vomiting within 36 h, the operation time, the intraoperative blood loss, the hospital stay, the postoperative exhaust time and bowel sounds recovery time were compared in the two groups. Results The occurrence of nausea and vomiting of level I within 36 h in the observation group was significantly higher than that of the control group, and vomiting of level II was lower than that of the control group （P〈0.05）; the operation time, intraoperative blood loss, hospital stay in the observation group were significantly better than those in the control group （P〈0.05）; the total effective rate of the observation group was higher than that of the control group （P〈0.05）; the postoperative exhaust time and bowel sounds recovery time of the observation group were significantly better than those of control group （P〈0.05）. Conclusion Preoperative TCM Congrong tongbian soup can significantly improve the clinical effect of laparoscopic cholecystectomy, it is worthy of clinical popularization and application.
