彩色多普勒超声对肝移植术后门静脉病变的检测价值

摘　　要：目的探讨彩色多普勒超声检测肝移植术后门静脉病变及其临床价值。方法肝移植患者术后超声发现门静脉病变27例为并发症组，包括19例门静脉血栓，5例门静脉吻合口狭窄，3例门静脉主干闭塞患者；27例肝移植后排除任何术后严重并发症者为无并发症组；选择30例健康志愿者为对照组，分别观察三组间肝动脉和肝静脉血流动力学差别，对所得参数进行统计学分析。结果与正常对照组及无并发症组比较，并发症组肝动脉内径增宽，最大流速增高，阻力指数降低（F=4．258、15．88、4．237，P〈0．05）。与正常对照组比较，无并发症组肝动脉内径及血流动力学改变差异未见统计学意义（P〉0．05）。三组间肝静脉流速变化差异未见统计学意义（P〉0．05），波动指数逐渐增加（F=8．257，P〈0．05）。结论彩色多普勒超声可以动态、直观显示移植肝门脉异常，对检测肝移植术后门静脉病变有重要临床价值。Objective To investigate the clinical value of color Doppler ultrasound on portal vein complications after liver transplantation. Methods Twenty-seven cases of portal vein disease in patients with liver transplantation were selected as portal vein disease group, including 19 cases of portal vein thrombosis, 5 cases of portal vein stenosis, 3 cases of portal vein trunk occlusion; 27 cases without complications after liver transplantation were selected as no portal vein disease group, 30 healthy volunteers were selected as control group. The hemodynamic differences among the 3 groups were observed. Results In the portal vein disease group, the liver artery diameter increased, the maximum flow velocity increased, the resistance index decreased （ F = 4. 258, 15. 88, 4. 237, P 〈 0. 05）. Compared with the normal control group, there was no significant difference in the diameter or hemodynamics of hepatic artery in the no portal vein disease group （P 〉 0. 05 ）. There was no obvious change in the hepatic venous flow （P 〉 0. 05 ）, the fluctuation index increased （ F = 8. 257, P 〈 0.05 ）. Conclusions Color Doppler ultrasound detection can dynamicly and directly show the abnomality of portal vein disease after liver transplantation, and important clinical value.
