肿瘤科护士在《癌因性疲乏护理指南》临床试点应用中的体验

摘　　要：目的了解肿瘤科护士在《癌因性疲乏护理指南》临床试点应用中的实践体验,反思临床应用过程,探讨指南临床应用的促进及阻碍因素,为类似的临床试点应用提供借鉴。方法采用质性研究对12名肿瘤科及放疗科护士进行半结构式访谈,并运用类属分析法进行资料的编码、分类、提取主题。结果应用初期,护士对开展指南临床试点应用的态度各异;应用过程中最主要的问题包括填表的负担、患者的理解力较弱和依从性差;应用过程的促进因素主要包括领导力的介入、患者重视癌因性疲乏的管理等;应用过程的阻碍因素主要包括护士人力资源相对短缺、流动性大,医生与护士沟通不足;整个应用过程中护士最主要的收获是更专业与更自信。结论在护理指南临床应用过程中,应重视护士在整个应用过程中的体验,分析其阻碍和促进因素,并制定针对性的干预策略,提升护士的认同感,以提高临床应用的效果。Objective To understand oncology nurses＆#39;experience in the implementation of clinical practice guideline on cancer related fatigue,reflect on the implementation of evidence into practice,to explore its fa-cilitation and barriers,and provide references for those similar clinical applications.Methods Semi-struc-tured interviews were conducted with 12 oncology and radiotherapy nursing staff,and the data were coded, classified and extracted by the method of class analysis.Results In the preliminary stage of the applica-tion,nurses＆#39;attitudes towards clinical practice guideline were varies.The main problems were the pressure of filling the forms,the weak perception and bad compliance of patients.The promoting factors were the involvement of the leadership,patients paid attention to the cancer related fatigue management.The main barriers were the shortage of nursing personnel,great mobility,lack of communication between doctors and nurses.The main out-comes were professional skills and confidence.Conclusions In the implementation of evidence to nursing practice, more attention should be paid to understand nurses＆#39;experiences,and to analyze the facilitation and barriers,formu-late specific interventions to enhance nurse＆#39;s identity and improve the effects of clinical application eventually.
